This is a descriptive retrospective and prospective
study of 101 patients admitted to Kenyatta National Hospital
(KNH) between 1st October, 1985 and 30th January, 1990 and
had bone marrow examination done by aspiration and trephine
needle biopsy. At KNH trephine needle bone marrow (TNBM)
biopsy has been performed over the years when aspiration
results in 'dry tap'. It is only available for the use by a
few specialists (haematologists) and it is rarely performed
in staging malignant lymphomas or as a routine diagnostic
technique in various other diseases. The study was done to
evaluate the role of the technique in patient care at this
hospital. The relevant data and the diagnostic outcome of
50 patients admitted to the hospital between 1st October,
1985 and 30th June, 1989 were collected retrospectively.
Trephine needle biopsies were performed by the investigator
on 51 patients during the last 7 months of the study. Data
collected included: the age and sex, the indications for
bone biopsy, the quality of the specimen, the reporting
format and the final diagnosis on the trephine biopsies.
The 101 patients studied were between 2 and 75 years of
age. The mean age was 23.9 years. There were 62 males and
39 females. A 'dry tap' aspirate, the commonest indication
for TNBM biopsy was reported in 37 (36.6%) cases. In twelve
cases, the aspirate and needle biopsy were performed at the
same time using the same needle. Ten of these were in the
staging of malignant lymphomas. Good or satisfactory
specimens were obtained in at least 86% of the biopsies

performed. Aplastic anaemia, the commonest abnormality
detected was found in 28 (27.7%) of the patients studied. A
review of the reporting format showed that in only 20% cases
in the retrospective study was a full report of the biopsy
given by the haematologist.
This study shows that TNBM biopsy is a simple and safe
procedure yielding a good or satisfactory specimen in most

instances. The biopsy will most likely provide a diagnosis
when bone marrow aspirate fails due to 'dry tap' or scanty
yield. The procedure may be of value in routine
investigation of various diseases such as aplastic anaemia
and in staging of malignant lymphomas although larger
studies need to be done in this area (only ten cases in this
study). A standard format should be formulated and adhered
to by haematologists and pathologists reporting on the TNBM
biopsies in this hospital.

Nasal nitric oxide (nNO) measurements are used in the assessment of patients suspected of having primary ciliary dyskinesia (PCD), but recommendations for performing such measurements have not focused on children and do not include all current practices. To guide the development of a European Respiratory Society-supported technical standard for nNO measurement in children, an international online survey was conducted to better understand current measurement practices among providers involved in PCD diagnostics. 78 professionals responded, representing 65 centres across 18 countries, mainly in Europe and North America. Nearly all centres measured nNO in children and more than half performed measurements before 5 years of age. The test was often postponed in children with signs of acute airway infection. In Europe, the electrochemical technique was more frequently used than chemiluminescence. A similar proportion of centres performed measurements during exhalation against a resistance (49 out of 65) or during tidal breathing (50 out of 65); 15 centres used only exhalation against a resistance and 15 used only tidal breathing. The cut-off values used to discriminate PCD were consistent across centres using chemiluminescence analysers; these centres reported results as an output (nL·min). Cut-off values were highly variable across centres using electrochemical devices, and nNO concentrations were typically reported as ppb. This survey is the first to determine real-world use of nNO measurements globally and revealed remarkable variability in methodology, equipment and interpretation. These findings will help standardise methods and training.

To ensure the long-term sustainable use of African Great Lakes (AGL), and to better understand the functioning of these ecosystems, authorities, managers and scientists need regularly collected scientific data and information of key environmental indicators over multi-years to make informed decisions. Monitoring is regularly conducted at some sites across AGL; while at others sites, it is rare or conducted irregularly in response to sporadic funding or short-term projects/studies. Managers and scientists working on the AGL thus often lack critical long-term data to evaluate and gauge ongoing changes. Hence, we propose a multi-lake approach to harmonize data collection modalities for better understanding of regional and global environmental impacts on AGL. Climate variability has had strong impacts on all AGL in the recent past. Although these lakes have specific characteristics, their limnological cycles show many similarities. Because different anthropogenic pressures take place at the different AGL, harmonized multi-lake monitoring will provide comparable data to address the main drivers of concern (climate versus regional anthropogenic impact). To realize harmonized long-term multi-lake monitoring, the approach will need: (1) support of a wide community of researchers and managers; (2) political goodwill towards a common goal for such monitoring; and (3) sufficient capacity (e.g., institutional, financial, human and logistic resources) for its implementation. This paper presents an assessment of the state of monitoring the AGL and possible approaches to realize a long-term, multi-lake harmonized monitoring strategy. Key parameters are proposed. The support of national and regional authorities is necessary as each AGL crosses international boundaries.

Paleoanthropologists have long speculated about the role of environmental change in shaping human evolution in Africa. In recent years, drill cores of late Neogene lacustrine sedimentary rocks have yielded valuable high-resolution records of climatic and ecosystem change. Eastern African Rift sediments (primarily lake beds) provide an extraordinary range of data in close proximity to important fossil hominin and archaeological sites, allowing critical study of hypotheses that connect environmental history and hominin evolution. We review recent drill-core studies spanning the Plio?Pleistocene boundary (an interval of hominin diversification, including the earliest members of our genus Homo and the oldest stone tools), and the Mid?Upper Pleistocene (spanning the origin of Homo sapiens in Africa and our early technological and dispersal history). Proposed drilling of Africa's oldest lakes promises to extend such records back to the late Miocene. ?High-resolution paleoenvironmental records are critical for understanding external drivers of human evolution. ?African lake basin drill cores play a critical role in enhancing hominin paleoenvironmental records given their continuity and proximity to key paleoanthropological sites. ?The oldest African lakes have the potential to reveal a comprehensive paleoenvironmental context for the entire late Neogene history of hominin evolution. The availability of water in the dryland regions is a function of climatic and land conditions, which determine the amount and distribution of atmospheric water and subsequent partitioning of that water into different hydrological components. Rainfall variability in the drylands affects water, food and livelihood security in the sensitive landscapes. For effective water resource management, it is important to quantify the effect of rainfall variability on the available water resource components to support food security, given that a majority of livelihoods in the drylands are dependent on rainfall. This study assessed rainfall variability and its effects on stream flows in the dryland regions of Turkana County in Northern Kenya. The Coefficient of Variation (CV), Standardised Precipitation Anomaly (SPA) and Mann-Kendal test approaches were used to determine variability and trends of rainfall. The Soil and Water Assessment Tool (SWAT), was employed to simulate hydrological components, including surface runoff, interflow, groundwater flow and the eventual water yield in the streams. The model simulations were driven by the Climate Hazards Group InfraRed Precipitation with Station CHIRPS and the Coordinated Regional Climate Downloading Experiment CORDEX projected rainfalls to establish the current and future water availability respectively. From the results, CV values ranging from 20 to 99% were obtained indicating that the region experiences moderate to high rainfall variability, often resulting in moderate to extreme floods and droughts in the area. The model simulations indicated that the stream-flows are increasing towards the 2020s decade compared to what was flowing in the 1980s decade, as observed between 1981 and 2020 using rainfall. This is probably a result of the increased rainfall observed in this region over the las four decades. Using Representative Concentration Pathways RCP 4.5 and RCP 8.5 projected rainfall scenarios, the model indicated that future stream flows will be higher than present. The findings show that, in the face of climate change scenarios, proper land management is required to promote water and food security in the drylands of sub-Saharan Africa in the future

The hydrogen isotopic composition of fossil leaf waxes (δDwax), assumed to record past variations in the hydrogen isotopic composition of precipitation (δDprecip), is increasingly being used to study past hydroclimatic change in Africa, and are commonly interpreted to reflect variation in the amount of precipitation through time (i.e., the amount effect). Although there are now many such δDprecip records from tropical Africa, there are few robust δDprecip records from easternmost equatorial Africa of sufficient length and resolution to evaluate the mechanisms governing hydroclimate variation during and since the LGM. Efficient management of construction works should focus its attention on utilizing labour, equipment, and raw materials in the right way. Besides, they claim that responsible parties should evaluate labour productivity continuously and that cost control measures should be developed. Low productivity witnessed in construction is linked to internal challenges including disputes, abandonment of projects, excess budget, and time overlaps. The objective of the study was to determine the financial effect of labour related risks on completion of PPP projects. A descriptive survey design was used with simple random sampling targeting 85 members of the management team at the completed Sondu-Miriu Power project. The participants were administered with questionnaire which were sent by mail (in some cases) and selected participants encouraged to respond. Data obtained was analysed through SPSS platform using descriptive statistics and Pearson Moment correlation as inferential statistics. The study found that labour related risks in construction PPP projects included failure by employees and experts to complete assigned tasks, injuries and deaths, changing labour demands during the project as well as cases of go slow. Construction labour related risks affected the financial cost, time and quality of work performed in the construction project. The study concludes that labour related risks have a financial effect on completion of PPP projects such that as the risks increase, the financial cost increases. The study recommends that more focus should be placed on the identification and recruitment of project staff as well task allocation.

Microplastic pollution has been recognised as a global threat in marine environments and a danger to prey, predators and humans. Yet, there have been few studies in the western Indian Ocean, specifically along the Kenyan coast, which makes it difficult to estimate the extent of such pollution in the region. This is the first study on microplastics (MPs) in the sediments within creeks and estuaries (Tudor, Port Reitz and Mida creeks) on the Kenyan coast. Abstract:
Food supply is less than food demand in Kitui County as population keeps increasing. This lack of food leads to
malnutrition, poor health and even death to human beings and animals both domestic and wild. Agriculture is the key
food source in Kitui County in the republic of Kenya. Occasionally residents buy food quantities from neighboring Arid
and semi-arid areas counties which is not a reliable source because they face similar challenge. Many interventions to
improve food security has been put in place and one such is irrigation by the local county and National government
initiated through National irrigation Board but sustainability of these projects is a great concern in Kitui County.
Despite the irrigation projects setup still biting is food insufficiency which persists. One approach to improve
sustainability of irrigation projects is carrying out participatory monitoring and evaluation and then taking
corrective action which could also lead to ownership and then improved sustainability. This was the objective of the
study which was to assess if taking corrective action after Participatory Monitoring and evaluation (PME)influence
project sustainability. The study used descriptive survey and correlation designs in order to collect data from 316
respondents who were selected using stratification, judgmental and purposeful with strict randomization methods.
Questionnaires were administered and interviews were conducted on selected respondents on appointed dates. Data
was analyzed using Statistical Package for Social scientist (SPSS) version 25.0in order to get descriptive statistics also
correlation coefficients were got in order to get association and degree of strength. Testing of hypothesis was done
using linear regression. The study findings show thata big percentage (56.3%) of the respondents were aged between
31 to 40years.Majority of the respondents were females and their highest-level of education was up to primary school.
The respondents agreed that PME data collection and taking corrective action were not done. Taking corrective
action had a weak influence on sustainability with a correlation coefficient (r)of 0.33 which only explains 10.9% of
project sustainability. It’s recommended that farmers should be trained more through seminars and vocational
institutions in order to empower them with skills. Also, farmers should be encouraged to participate in irrigation
projects as they can have a sense of ownership. Infrastructure should be improved in order to access markets and a PM
& Eunit should be set up in irrigation projects in order to spear data collection and analysis and guide projects
towards sustainability. This is significant because it will improve sustainability and thus more food supply reducing
malnutrition rates and death among people and animals.

Online learning has increasingly been adopted by most institutions of higher learning to facilitate teaching and learning as a continuum to the traditional face-to-face approach. Most of these institutions utilize Learning Management Systems which contain features that are intended to make students active participants not only by delivering learning resources to learners but also providing the environment for effective interaction in the learning process. Our examination of the literature reveals that there is limited empirical evidence that addresses how these features are being utilized by students in promoting Self-Regulated learning. To realize the usefulness of the features of Learning Management Systems in promoting Self-Regulated Learning, a structured survey was carried out among University students in Kenya. The findings reveal that the features of Learning Management Systems are underutilized by students. The qualitative results of the study illustrate that students face several challenges that obstruct them from being actively involved in online learning. There is lack of individualized feedback on students' learning habits, lack of instructor guidance, lack of interaction with course instructors, lack of peer interaction and lack of automation tools. This study provides insights for educators and researchers on the areas of focus that can be prioritized towards offering support to students in improving their Self-Regulated learning in online learning environments.

Taenia species of domestic dogs can cause cysticercosis and coenurosis in a wide range of intermediate hosts including humans. Most taeniids of dogs are globally distributed, but some wildlife-transmitted species can be specific for certain regions. Generally, little information exists on the species composition and frequency in most regions of the world, which impairs risk assessment and control strategies. This study determined the range of taeniid species in dogs in four widely spaced areas of Kenya by genetic identification of eggs in faeces collected from the environment. Individual taeniid eggs were characterised by nested polymerase chain reaction of NADH dehydrogenase subunit 1 and cytochrome C oxidase 1 genes, restriction fragment length polymorphism and partial sequencing. Overall 79/1621 (4.9%) faecal samples contained eggs of Taenia or Hydatigera (8.0% in Turkana, 4.8% in Isiolo, 3.8% in Maasai Mara and 1.3% in Meru). Taenia hydatigena and T. multiceps were the most frequent, found in 36 and 15 samples, respectively. Other eggs found in the faeces belonged to T. serialis (sensu lato), T. madoquae (the first record in domestic dogs), T. ovis, T. saginata and Hydatigera taeniaeformis. Polymorphism of nad1 sequences revealed 22 and 8 haplotypes of T. hydatigena and T. multiceps, respectively. The results show the involvement of dogs in both domestic and sylvatic transmission cycles. In addition to the species range, this study provides data on the intraspecific diversity of T. hydatigena and T. multiceps in Kenya, which will serve as baseline information for further studies into cysticercosis and coenurosis in livestock and humans in the region.

Cataract is the leading cause of blindness globally and a major cause of vision impairment. Cataract surgery is an efficacious intervention that usually restores vision. Although it is one of the most commonly conducted surgical interventions worldwide, good quality services (from being detected with operable cataract to undergoing surgery and receiving postoperative care) are not universally accessible. Poor quality understandably reduces the willingness of people with operable cataract to undergo surgery. Therefore, it is critical to improve the quality of care to subsequently reduce vision loss from cataract. This scoping review aims to summarise the nature and extent of the published literature on interventions to improve the quality of services for primary age-related cataract globally.

We assessed 1) expansions of chemosensory protein orthologs in Glossina morsitans morsitans, Glossina pallidipes, Glossina austeni, Glossina palpalis gambiensis, Glossina fuscipes fuscipes and Glossina brevipalpis tsetse fly species using Café analysis (to identify species-specific expansions) and 2) differential expressions of the orthologs and associated proteins in male G. m. morsitans antennae and head tissues using RNA-Seq approaches (to establish associated functional molecular pathways). We established accelerated and significant (P<0.05, λ = 2.60452e-7) expansions of gene families in G. m. morsitans Odorant receptor (Or)71a, Or46a, Ir75a,d, Ionotropic receptor (Ir) 31a, Ir84a, Ir64a and Odorant binding protein (Obp) 83a-b), G. pallidipes Or67a,c, Or49a, Or92a, Or85b-c,f and Obp73a, G. f. fuscipes Ir21a, Gustatory receptor (Gr) 21a and Gr63a), G. p. gambiensis clumsy, Ir25a and Ir8a, and G. brevipalpis Ir68a and missing orthologs in each tsetse fly species. Most abundantly expressed transcripts in male G. m. morsitans included specific Or (Orco, Or56a, 65a-c, Or47b, Or67b, GMOY012254, GMOY009475, and GMOY006265), Gr (Gr21a, Gr63a, GMOY013297 and GMOY013298), Ir (Ir8a, Ir25a and Ir41a) and Obp (Obp19a, lush, Obp28a, Obp83a-b Obp44a, GMOY012275 and GMOY013254) orthologs. Most enriched biological processes in the head were associated with vision, muscle activity and neuropeptide regulations, amino acid/nucleotide metabolism and circulatory system processes. Antennal enrichments (>90% of chemosensory transcripts) included cilium-associated mechanoreceptors, chemo-sensation, neuronal controlled growth/differentiation and regeneration/responses to stress. The expanded and tsetse fly species specific orthologs includes those associated with known tsetse fly responsive ligands (4-methyl phenol, 4-propyl phenol, acetic acid, butanol and carbon dioxide) and potential tsetse fly species-specific responsive ligands (2-oxopentanoic acid, phenylacetaldehyde, hydroxycinnamic acid, 2-heptanone, caffeine, geosmin, DEET and (cVA) pheromone). Some of the orthologs can potentially modulate several tsetse fly species-specific behavioral (male-male courtship, hunger/host seeking, cool avoidance, hygrosensory and feeding) phenotypes. The putative tsetse fly specific chemosensory gene orthologs and their respective ligands provide candidate gene targets and kairomones for respective downstream functional genomic and field evaluations that can effectively expand toolbox of species-specific tsetse fly attractants, repellents and other tsetse fly behavioral modulators.

Background: People living with diabetes can reduce their risk of vision loss from diabetic retinopathy by attending screening, which enables early detection and timely treatment. The aim of this pilot trial was to assess the feasibility of a full-scale cluster randomized controlled trial of an intervention to increase uptake of retinal examination in this population, as delivered within existing community-based diabetes support groups (DSGs).

Methods: All 16 DSGs in Kirinyaga county were invited to participate in the study. The first two groups recruited took part in the pilot trial. DSG members who met the eligibility criteria were recruited before the groups that were randomized to the two arms. In the intervention group, two peer educators were trained to deliver monthly DSG-based eye health education and individual telephone reminders to attend screening. The control group continued with usual DSG practice which is monthly meetings without eye health education. The recruitment team and outcome assessors were masked to the allocation. We documented the study processes to ascertain the feasibility, acceptability, and potential effectiveness of the intervention. Feasibility was assessed in terms of clarity of study procedures, recruitment and retention rates, level of acceptability, and rates of uptake of eye examination. We set the target feasibility criteria for continuation to the main study to be recruitment of 50 participants in the trial, 80% monthly follow-up rates for individuals, and no attrition of clusters.

Results: Of the 122 DSG members who were assessed for eligibility, 104 were recruited and followed up: 51 (intervention) and 53 (control) arm. The study procedures were well understood and easy to apply. We learnt the DSG meeting days were the best opportunities for recruitment. The study had a high acceptance rate (100% for clusters, 95% for participants) and high follow-up and retention rate (100% of those recruited). All clusters and participants were analysed. We observed that the rate of incidence of eye exam was about 6 times higher in the intervention arm as compared to the control arm. No adverse unexpected events were reported in either arm.

Conclusions: The study is feasible and acceptable in the study population. The results support the development of a full-scale cluster RCT, as the success criteria for the pilot were met.

Background: There is limited evidence on how implementation of peer support interventions influences effectiveness, particularly for individuals with diabetes. We conducted a cluster randomized controlled trial to compare the effectiveness of a peer-led health education package versus usual care to increase uptake of screening for diabetic retinopathy (DR).

Methods: Our process evaluation used a mixed-method design to investigate the recruitment and retention, reach, dose, fidelity, acceptability, and context of implementation, and was guided by the Consolidated Framework for Implementation Research (CFIR). We reviewed trial documents, conducted semi-structured interviews with key informants (n = 10) and conducted four focus group discussions with participants in both arms of the trial. Three analysts undertook CFIR theory-driven content analysis of the qualitative data. Quantitative data was analyzed to provide descriptive statistics relevant to the objectives of the process evaluation.

Results: The trial had positive implementation outcomes, 100% retention of clusters and 96% retention for participants, 83% adherence to delivery of content of group talks (fidelity), and 78% attendance (reach) to at least 50% (3/6) of the group talks (dose). The data revealed that intervention characteristics, outer setting, inner setting, individual characteristics, and process (all the constructs of CFIR) influenced the implementation. There were more facilitators than barriers to the implementation. Facilitators included the relative advantage of the intervention compared with current practice (intervention characteristics); awareness of the growing prioritization of diabetes in the national health policy framework (outer setting); tension for change due to the realization of the vulnerability to vision loss from DR (inner setting); a strong collective sense of accountability of peer supporters to implement the intervention (individual characteristics); and regular feedback on the progress with implementation (process). Potential barriers included the need to queue at the eye clinic (intervention characteristic), travel inconveniences (inner setting), and socio-political disruption (outer setting).

Conclusions: The intervention was implemented with high retention, reach, fidelity, and dose. The CFIR provided a valuable framework for evaluating contextual factors that influenced implementation and helped to understand what adaptations may be needed during scale up.

Fluorescent natural organic matter at tryptophan-like (TLF) and humic-like fluorescence (HLF) peaks is associated with the presence and enumeration of faecal indicator bacteria in groundwater. We hypothesise, however, that it is predominantly extracellular material that fluoresces at these wavelengths, not bacterial cells. We quantified total (unfiltered) and extracellular (filtered at < 0.22 µm) TLF and HLF in 140 groundwater sources across a range of urban population densities in Kenya, Malawi, Senegal, and Uganda. Where changes in fluorescence occurred following filtration they were correlated with potential controlling variables. A significant reduction in TLF following filtration (ΔTLF) was observed across the entire dataset, although the majority of the signal remained and thus considered extracellular (median 96.9%). ΔTLF was only significant in more urbanised study areas where TLF was greatest. Beneath Dakar, Senegal, ΔTLF was significantly correlated to total bacterial cells (ρs 0.51). No significant change in HLF following filtration across all data indicates these fluorophores are extracellular. Our results suggest that TLF and HLF are more mobile than faecal indicator bacteria and larger pathogens in groundwater, as the predominantly extracellular fluorophores are less prone to straining. Consequently, TLF/HLF are more precautionary indicators of microbial risks than faecal indicator bacteria in groundwater-derived drinking water.

Abstract: health-related problems and even death among animals and human beings. Agriculture is the main food source; thus, many interventions are made such as that of irrigation by the local county and national government initiated through the National Irrigation Board (NIB). Despite the irrigation projects food insufficiency still persists, therefore their sustainability is questionable. One such approach to improving the sustainability of irrigation projects is participatory monitoring and evaluation which leads to ownership and then higher sustainability. In the study, the objective was to asses if taking corrective action after participatory monitoring and evaluation (PME) influence project sustainability. The study used a descriptive survey and correlation designs to collect data from 316 respondents selected using stratification sand purposeful with strict randomization. Questionnaires were administered and interviews were conducted on selected sample respondents on appointed dates. Data were analyzed using SPSS version 25.0 to get descriptive statistics, correlations coefficients were obtained to test association and degree of strength. Testing of the hypothesis was done using linear regression. The study findings were that a large number of respondents were between ages 31 to 40 years and most were female with their highest level of education being primary school. The influence of PME capacity building on the dependent variable and irrigation projects sustainability found that the farmers were not taken for exposure visits and project officers were not accountable for money use. Age, gender, and education level have very minimal influence on PME capacity building. PME capacity building had a weak positive influence of r = 0.290 and it explained only 8.4% of irrigation projects sustainability in Kitui County. The study recommends that to improve project capacity building: project revenue must be controlled on use, farmers must be taken for exposure visits to learn from successors, project officers should be accountable for funds use, and project guidelines should be improved to increase sustainability. Implementation of these recommendations will reduce the loss of Arid and Semi-Srid Lands (ASALs) and attain higher and longer sustainability in food projects, thus, reducing the recurrence rate of food shortage, improve and hasten the implementation of irrigation projects, show the need to involve primary stakeholders in project monitoring and appraisal for sustainability, better and efficient decisions by policymakers to increase chances of project's success. The study employed the use of pragmatism paradigm and a descriptive research design to collect both quantitative and qualitative data. A correlational design was used to test the hypothesis. A sample of 221 respondents was selected from a population of 517 using stratified random sampling. Data was collected using a structured questionnaire with both open and close ended with Likert type on 1-5 five point scale and interview schedule. Correlational design was used to test the hypothesis. Quantitative data was analyzed using descriptive and inferential statistics and data presented in frequency tables using means and standard deviations while qualitative data was presented in in narrative statements. Hypothesis was tested using linear regression at 0.05 level of significance to determine the degree and direction of relationships among the thematic area of the study. The reliability of the instruments was obtained using Cronbach Alpha coefficient of 0.735 for all items meaning that the instruments were reliable. The results showed that data dissemination and use and provision of curative and preventive tuberculosis health care services in public health institutions had [r2=0.186, p<0.05]. The results for quantitative data based on composite mean score of 3.91 and 0.466 standard deviation for this variable respectively. Therefore, data dissemination and use influence provision of curative and preventive tuberculosis healthcare services in public health institutions.

Objective To evaluate changes in awareness of maternal sepsis among healthcare providers resulting from the WHO Global Maternal Sepsis Study (GLOSS) awareness campaign.

Design Independent sample precampaign/postcampaign through online and paper-based surveys available for over 30 days before campaign roll-out (pre) and after study data collection (post). Descriptive statistics were used for campaign recognition and exposure, and odds ratio (OR) and percentage change were calculated for differences in awareness, adjusting for confounders using multivariate logistic regression.

Setting and participants Healthcare providers from 398 participating facilities in 46 low, middle and high-income countries.

Intervention An awareness campaign to accompany GLOSS launched 3 weeks prior to data collection and lasting the entire study period (28 November 2017 to 15 January 2018) and beyond.

Results A total of 2188 surveys were analysed: 1155 at baseline and 1033 at postcampaign. Most survey respondents found the campaign materials helpful (94%), that they helped increase awareness (90%) and that they helped motivate to act differently (88%). There were significant changes with regard to: not having heard of maternal sepsis (−63.4% change, pre-OR/post-OR 0.35, 95% CI 0.18 to 0.68) and perception of confidence in making the right decisions with regard to maternal sepsis identification and management (7.3% change, pre-OR/post-OR 1.44, 95% CI 1.01 to 2.06).

Conclusions Awareness raising campaigns can contribute to an increase in having heard of maternal sepsis and an increase in provider perception of confidence in making correct decisions. Offering the information to make accurate and timely decisions while promoting environments that enable self-confidence and support could improve maternal sepsis identification and management.

We formulate a sex-structured deterministic model to study the effects of varying HIV testing rates, condom use rates and ART adherence rates among Adolescent Girls and Young Women (AGYW) and, Adolescent Boys and Young Men (ABYM) populations in Kenya. Attitudes influencing the Kenyan youth HIV/AIDS control measures both positively and negatively were considered. Using the 2012 Kenya AIDS Indicator Survey (KAIS) microdata we constructed our model, which we fitted to the UNAIDS-Kenya youth prevalence estimates to understand factors influencing Kenyan youth HIV/AIDS prevalence trends. While highly efficacious combination control approach significantly reduces HIV/AIDS prevalence rates among the youth, the disease remains endemic provided infected unaware sexual interactions persist. Disproportional gender-wise attitudes towards HIV/AIDS control measures play a key role in reducing the Kenyan youth HIV/AIDS prevalence trends. The female youth HIV/AIDS prevalence trend seems to be directly linked to increased male infectivity with decreased female infectivity while the male youth prevalence trend seems to be directly associated with increased female infectivity and reduced male infectivity.

Microplastic pollution has been recognized as a global threat in marine environments and a danger to prey, predators and humans. Yet there have been limited studies in the Western Indian Ocean (WIO) and along the Kenyan coast making it difficult to estimate the extent of such pollution. This is the first study on microplastics (MPs) in the surface waters within creeks (Tudor, Port-Reitz and Mida creeks) in Kenya. Sampling was done in January/February and September 2018 to collect microplastics from surface water. Neuston nets of 500 μm (large) and 250 μm (medium) size were towed for ten minutes and 50 litres of seawater sieved through a 20 μm net (small) in three replicates. The samples were digested in 10% Potassium Hydroxide, sieved, and then filtered with cellulose nitrate membrane microfilters. Concentrations of total microplastics, different shapes and colours were established under a microscope. High concentrations of small size (20-250 μm) MPs were encountered and Tudor and Port Reitz had higher concentrations compared to Mida Creek. The study provides data on microplastic concentrations within the creeks and recommends focussing on small size microplastics for monitoring purposes, which due to their high concentrations can be hazardous to organisms.

Mathematical analysis of infection through sub-network analysis was carried out to trace various infection routes and the veracity of various transmission routes as well as the associated probabilities. Using HIV data in Kenya on our model, disproportional effects were observed when dispensation of comprehensive knowledge of HIV/AIDS was preferred in one population over the other. Effective dispensation of comprehensive knowledge of HIV/AIDS in both the infected AGYW and ABYM populations significantly slows down the infection spread but may not eradicate it.

This research was focused on assessing how monitoring and evaluation work plan influence provision of curative and preventive tuberculosis healthcare practices in institutions of public health in Kisumu County, Kenya. The study unit of analysis was public health institutions that practice M&E system on provision of health care services. The target population consisting of doctors, M&E officers, clinical officers, nurses and patients in four public health institutions in Kisumu county, Kenya. The study was guided by pragmatism paradigm. A descriptive survey research design was employed to collect both quantitative and qualitative data and correlational design was used to test the hypothesis. A sample of 221 respondents was selected from a population of 517 using stratified random sampling. A structured questionnaire with both open and close ended with Likert type on 1-5 five point scale and interview schedule was used to collect data. A descriptive survey research design was employed to collect both quantitative and qualitative data and correlational design was used to test the hypothesis. Quantitative data was analyzed using descriptive and inferential statistics and data presented in frequency tables using means and standard deviations while qualitative data was presented in in narrative statements. Hypothesis was tested using linear regression at 0.05 level of significance to determine the degree and direction of relationships among variables. The study attained reliability of instruments using Cronbach Alpha coefficient of 0.735 for all items implying that the instrument was reliable. The results indicated that showed that M&E work plan regression was (r2=0.028, p<0.05). The study results indicated that present results for quantitative data based on composite mean score of 3.90 and 0.447 standard deviation for this variable. M&E system therefore, contributes significantly to the effective provision of curative and preventive tuberculosis health care services in public health institutions

This study sought to determine nutrition knowledge and its association with practices in management of stage 5 chronic
kidney disease (CKD) among renal adult patients in Kenyatta National Hospital. A cross-sectional analytical design was used to
facilitate collection of qualitative and quantitative data and enable identification of associations between variables. Cochran formula
was used to calculate a sample of 110 respondents. A research administered questionnaire with closed ended questions was used to
collect information. Descriptive statistics such as frequency and percentage were used to describe demographic and socio-economic
characteristics of the population. Inferential statistics were done using chi square with an alpha of 0.05 to test association between
independent and dependent variables. Most participants had moderate knowledge levels whereas more than half of the participants
(66%) indicated poor nutrition management practices of their condition. There was significant association between period on dialysis
in months and practices in management of stage 5 chronic kidney disease. Knowledge on importance of diet in management of stage 5
CKD (<0.001), moderation of fluid intake (<0.001), reduction in salt/ sodium intake (<0.001), and moderation in protein intake
(<0.004) were significantly associated with nutritional management practices of stage 5 CKD. The study concluded that Knowledge
levels were low in identifying foods rich in phosphorus, calcium and fat soluble vitamins. Practice scores were also poor despite
moderate nutrition knowledge levels. Patients’ knowledge on nutrition management of their condition may be achieved through
frequent and consistent nutrition education and counseling including follow ups.

Five new compounds—rhodimer (1), rhodiflavan A (2), rhodiflavan B (3), rhodiflavan C (4), and rhodacarpin (5)—along with 16 known secondary metabolites, were isolated from the CH2Cl2–CH3OH (1:1) extract of the roots of Tephrosia rhodesica. They were identified by NMR spectroscopic, mass spectrometric, X-ray crystallographic, and ECD spectroscopic analyses. The crude extract and the isolated compounds 2–5, 9, 15, and 21 showed activity (100% at 10 μg and IC50 = 5–15 μM) against the chloroquine-sensitive (3D7) strain of Plasmodium falciparum.

Aims: As shown in literature, several authors have adopted various individual frailty mixing distributions as a way of dealing with possible heterogeneity due to unobserved covariates in a group of insurers. This research contribution is to generalize the frailty mixing distribution to nest other classes of frailty distributions not in literature and apply the proposed distributions in valuation of life annuity business.

Methodology: A simulation study is done to assess the performance of the aforementioned models. The baseline parameters is estimated using Bayesian Inference and a better model is suggested for valuation of life annuity business.

Results: As a result of generalizing the frailty some new classes of frailty distributions are constructed such as; the Reciprocal Inverse Gaussian Frailty, the Inverse Gamma Frailty, the Harmonic Frailty and the Positive Hyperbolic Frailty.

From the simulation study, the proposed new frailty models shows that ignoring frailty leads to an underestimation of future residual lifetime since the survival curve shifts to the right when heterogeneity is accounted for. This is consistent with frailty literature.

The Reciprocal Inverse Gaussian model closely represents the Association of Kenya Insurers graduated rates with a slight increase in survival due to longevity risk.

Conclusion: The proposed new frailty models show an increase in the insurers expected liability when unobserved heterogeneity is accounted for. This is consistent with frailty literature and thus can be applied to avoid underestimating the insurer’s liability in the context of life annuity business.

The RIG model as proposed in estimating future liability by directly adjusting the AKI mortality rates shows an increase in longevity risk. The extent of heterogeneity of the insured group determines the level of risk. The RIG frailties should be considered for multivariate cases where the insureds are clustered in groups.

Due to the widespread calls for the construction industry to adopt sustainable approaches, the various stakeholders are now engaging in the sustainability agenda more than before. This study investigated how the Kenyan construction industry is engaging the sustainability agenda. Specifically, this study sought to establish sustainable construction (SC) literacy levels, key sustainability considerations and SC literacy avenues in the interior design market segment of the Kenyan construction industry. Key project stakeholders in the interior design market segment of the Kenyan construction industry were the target population. A total of 60 (12 architects/interior designers, 12 electrical engineers, 12 mechanical engineers,12 quantity surveyors and 12 contractors) structured questionnaires were distributed, out of which 46 (10 architects/interior designers, 9 electrical engineers, 9 mechanical engineers, 8 quantity surveyors and 10 contractors) were received back. Collected data was analysed using frequencies, percentages, mean item scores (MIS) and standard deviations (SD). The study revealed an average level of sustainability literacy with a composite mean score of 3.7102 and mismatch between SC literacy levels and key sustainability considerations in interior design projects. Additionally, the respondents rated standard SC approaches, legislation, policies and construction trade associations as the least effective contributors to their current SC literacy levels. On the other hand, informal learning, construction professional associations influence, collaboration amongst firms, and formal learning were largely attributed to the respondent’s SC literacy levels. The implication of the findings was that there is need to fine-tune SC literacy drives to the peculiarities of the various industry market segments to ensure their effectiveness in informing practice. Additionally, there is the need to leverage standard SC approaches, legislation, policies and construction trade associations as avenues to improve the overall sustainability literacy levels.

Construction industry has been identified as key to the sustainability agenda. Efforts towards improved sustainability compliance in the construction industry involve a socio-technical transformation. Such transitions are said to be purposive, of a wider perspective, multi-dimensional in nature and are influenced by numerous factors. This paper focused on establishing sustainable construction (SC) uptake levels, including identification of key SC drivers and barriers with specific reference to the interior design market segment of the Kenyan construction industry. The study employed a quantitative research approach. For quantitative attributes, the study employed structured questionnaires to collect data from actively practicing architects/interior designers, electrical engineers, mechanical engineers, quantity surveyors and contractors drawn from the interior design market segment of Kenyan construction industry in Nairobi City County. Data analysis employed the descriptive statistics of distribution (frequencies), proportions (percentages), central tendency (mean) and dispersion (standard deviation). Generally, the respondents ranked the overall uptake of the three dimensions of sustainability (economic, environmental and social) as average- ranking as social, environment and economic in a decreasing order of uptake levels. The study findings ranked sustainability driver categories as organization related drivers, stakeholder related drivers, economic related drivers and management related drivers; in order of decreasing influence. Lastly, barrier categories were ranked as economic related barriers, professional/capacity related barriers, technology related barriers and societal/cultural related barriers, in order of decreasing influence. With the average sustainable construction practices uptake in the Kenyan construction industry, there is an implied call to action to leverage known sustainability drivers, while at the same time suppressing the known barriers. This implies significant room for improvement and an appropriate starting point for key construction project stakeholders as above identified.

Introduction
The World Health Organization’s (WHO) Labour Care Guide (LCG) is a “next‐generation” partograph based on WHO’s latest intrapartum care recommendations. It aims to optimize clinical care provided to women and their experience of care. We evaluated the LCG’s usability, feasibility, and acceptability among maternity care practitioners in clinical settings.

Methods
Mixed‐methods evaluation with doctors, midwives, and nurses in 12 health facilities across Argentina, India, Kenya, Malawi, Nigeria, and Tanzania. Purposively sampled and trained practitioners applied the LCG in low‐risk women during labor and rated experiences, satisfaction, and usability. Practitioners were invited to focus group discussions (FGDs) to share experiences and perceptions of the LCG, which were subjected to framework analysis.

Results
One hundred and thirty‐six practitioners applied the LCG in managing labor and birth of 1,226 low‐risk women. The majority of women had a spontaneous vaginal birth (91.6%); two cases of intrapartum stillbirths (1.63 per 1000 births) occurred. Practitioner satisfaction with the LCG was high, and median usability score was 67.5%. Practitioners described the LCG as supporting precise and meticulous monitoring during labor, encouraging critical thinking in labor management, and improving the provision of woman‐centered care.

Conclusions
The LCG is feasible and acceptable to use across different clinical settings and can promote woman‐centered care, though some design improvements would benefit usability. Implementing the LCG needs to be accompanied by training and supportive supervision, and strategies to promote an enabling environment (including updated policies on supportive care interventions, and ensuring essential equipment is available).

BACKGROUND: To determine the prevalence and causes of blindness and vision impairment (VI) in East Asia in 2015 and to forecast the trend to 2020.
METHODS: Through a systematic literature review and meta-analysis, we estimated prevalence of blindness (presenting visual acuity <3/60 in the better eye), moderate-to-severe vision impairment (MSVI; 3/60≤presenting visual acuity <6/18), mild vision impairment (mild VI: 6/18≤presenting visual acuity <6/12) and uncorrected presbyopia for 1990, 2010, 2015 and 2020. A total of 44 population-based studies were included.
RESULTS: In 2015, age-standardised prevalence of blindness, MSVI, mild VI and uncorrected presbyopia was 0.37% (80% uncertainty interval (UI) 0.12%-0.68%), 3.06% (80% UI 1.35%-5.16%) and 2.65% (80% UI 0.92%-4.91%), 32.91% (80% UI 18.72%-48.47%), respectively, in East Asia. Cataract was the leading cause of blindness (43.6%), followed by uncorrected refractive error (12.9%), glaucoma, age-related macular degeneration, corneal diseases, trachoma and diabetic retinopathy (DR). The leading cause for MSVI was uncorrected refractive error, followed by cataract, age-related macular degeneration, glaucoma, corneal disease, trachoma and DR. The burden of VI due to uncorrected refractive error, cataracts, glaucoma and DR has continued to rise over the decades reported.
CONCLUSIONS:Addressing the public healthcare barriers for cataract and uncorrected refractive error can help eliminate almost 57% of all blindness cases in this region. Therefore, public healthcare efforts should be focused on effective screening and effective patient education, with access to high-quality healthcare.

Globally, eye care provision is currently insufficient to meet the requirement for eye care services. Lack of access and awareness are key barriers to specialist services; in addition, specialist services are over-utilised by people with conditions that could be managed in the community or primary care. In combination, these lead to a large unmet need for eye health provision. We have developed a validated smartphone-based screening algorithm (Peek Community Screening App). The application (App) is part of the Peek Community Eye Health system (Peek CEH) that enables Community Volunteers (CV) to make referral decisions about patients with eye problems. It generates referrals, automated short messages service (SMS) notifications to patients or guardians and has a program dashboard for visualising service delivery. We hypothesise that a greater proportion of people with eye problems will be identified using the Peek CEH system and that there will be increased uptake of referrals, compared to those identified and referred using the current community screening approaches.

Tsetse flies (Glossina sp.) are the vectors of human and animal trypanosomiasis throughout sub-Saharan Africa. Tsetse flies are distinguished from other Diptera by unique adaptations, including lactation and the birthing of live young (obligate viviparity), a vertebrate blood-specific diet by both sexes, and obligate bacterial symbiosis. This work describes the comparative analysis of six Glossina genomes representing three sub-genera: Morsitans (G. morsitans morsitans, G. pallidipes, G. austeni), Palpalis (G. palpalis, G. fuscipes), and Fusca (G. brevipalpis) which represent different habitats, host preferences, and vectorial capacity.
