Twenty variceal banding sessions were performed in eight patients between February 1995 and September 1996. A total of 69 rings were used to band the varices and at each session between two to six rings were used. Two of the eight had active bleeding and both underwent variceal banding to successfully arrest their bleeding as inpatients. Sixteen other variceal banding sessions were performed on an outpatient basis to obliterate their varices. Four of the eight patients had had sclerotherapy before and varices were still present. No acute or long term complications were noted. In one patient, variceal banding could not be performed as he developed stridor upon placement of the overtube. All the patients had advanced varices (Grade III or IV) and extended for more than 15 cms in the oesophagus. Endoscopic variceal obliteration remains the treatment of choice for patients with portal hypertension with variceal bleeding. Variceal banding is associated with a superior outcome when compared with sclerotherapy; the variceal kill time is shorter, infective complications less, rebleeding occurs less commonly and transfusion requirements are lower.

Abstract
Kenyan communities are exposed to natural disasters by an amalgamation of factors such as poverty, aridity, and settlements in areas susceptible to natural disasters or in areas with poor infrastructure. This is expected to increase due to the effects of climate change. In an attempt to explain some of these variabilities, we model the extreme damages from natural disasters in Kenya by developing a compound distribution that takes into account both the frequency and the severity of the extreme events. The resulting distribution is based on a threshold model and compound extreme value distribution. For frequency of events exceeding a threshold of 150,000, we found that it follows a negative binomial distribution, while severity of exceedance follows a generalized Pareto distribution. This distribution fits the data well and is found to be a better model for natural disasters in Kenya than the traditional extreme value threshold model.

MUTUKU DRNZIMBIBERNARD. On Decomposition of Operators in Hilbert Spaces. Nairobi: University of Nairobi; 1999. Abstract

The almost-similar and similar relations between operators on finite-dimensional Hilbert spaces are investigated. It is shown that almost-similar operators share some properties with some other classes of operators. Various results on almost-similarity and similarity are proved. An attempt is made to classify those operators where almost-similarity implies similarity. We investigate some properties of corresponding parts of operators which enjoy these equivalence relations.

Equi-transmitting scattering matrices are studied. A complete description of such matrices up to order five is given. It is shown that the standard matching conditions matrix is essentially the only equi-transmitting matrix for orders 3 and 5. For orders 4 and 6, there exists other equi-transmitting ones but all such matrices have zero trace

In this research paper we do an introduction to Hilbert space frames. We also discuss various frames in the Hilbert space. A frame is a generalization of a basis. It is useful, for example, in signal processing. It also allows us to expand Hilbert space vectors in terms of a set of other vectors that satisfy a certain condition. This condition guarantees that any vector in the Hilbert space can be reconstructed in a numerically stable way from its frame coe? cients. Our focus will be on frames in? nite dimensional spaces.

In this paper, we consider generalised distributions in the context of modelling dispersion but with focus on probability generating function (pgf) which is an important tool in studying statistical properties of a discrete distribution. The aim of this paper is twofold, one is to demonstrate the power of generalising in determination of pgf and two is to show that relationship between power series can naturally lead to pgf of a generalised distribution. Generalised Poisson distributions such as negative binomial, Pólya-Aeppli and Neyman type A are used to model overdispersed (clustered) populations and they all have Poisson as a limiting distribution as contagion breaks down to randomness. In particular, the Pólya-Aeppli distribution served as a typical example in underpinning the inherent power of generalising in determining the pgf. Based on the power series distributions, it is affirmed that negative binomial distribution is a generalised Poisson distribution by utilising the relationship between exponential,

A search for a distribution which adequately describes the dynamics of log returns has been a subject of study for many years. Empirical evidence has resulted in stylized facts of returns. Arguably, in this study, the three components of returns, mean equation part, the changing variance. part and the resulting residuals are determined and their corresponding parameters estimated within the proposed framework. Spectral density analysis is used to trace the seasonality component.

In this paper, nonparametric regression is employed which provides an estimation of unknown finite population
totals. A robust estimator of finite population totals in model based inference is constructed using the procedure of local linear
regression. In particular, robustness properties of the proposed estimator are derived and a brief comparison between the
performances of the derived estimator and some existing estimators is made in terms of bias, MSE and relative efficiency.
Results indicate that the local linear regression estimator is more efficient and performing better than the Horvitz-Thompson
and Dorfman estimators, regardless of whether the model is specified or mispecified. The local linear regression estimator also
outperforms the linear regression estimator in all the populations except when the population is linear. The confidence
intervals generated by the model based local linear regression method are much tighter than those generated by the design
based Horvitz-Thompson method. Generally the model based approach outperforms the design based approach regardless of
whether the underlying model is correctly specified or not but that effect decreases as the model variance increases.

O. PROFADUOLFRANCISW. "On outlier identification in geodetic networks using principal component analysis. International Conference on Influential Data Analysis,.". In: University of Sheffield, Sheffield, UK,. F.N. kamau, G. N Thothi and I.O Kibwage; Submitted. Abstract
A model for the establishment of an integrated geodetic datum covering a region is treated. The integrated geodetic datum is understood here to comprise the geoid, the reference ellipsoid, and the positional coordinates. A comprehensive unified solution model including the three parameters comprising the datum is considered. The estimation model adopted rigorously incorporates determined prior information in the estimation of the datum parameters. Mathematical models together with the pertinent observation equations are presented, followed by a description of the procedure for the aetual setting up of the datum. The observational scheme is based on a system of modular traverses anchored on a trilateration of GPS baselines.

This article develops, a lattice-based approach for pricing contingent claims when parameters governing the logs of the underlying asset dynamics are modelled by generalized hyperbolic distribution and normal inverse Gaussian distribution. The pentanomial lattice is constructed using a moment matching procedure. Moment generating functions of generalized hyperbolic distribution and normal inverse Gaussian distribution are utilized to compute probabilities and jump parameters under historical measure P. Minimal entropy martingale measure (MEMM) is used to value European call option with a view of comparing the results with some of the existing benchmark models such as Black Scholes model. Empirical data from S&P500 index, RUTSELL2000 index and RUI1000 index are used to demonstrate how the model works. There is a significant difference especially for long term maturity (six months and above) type of contracts, the proposed model outperform the benchmark model, while performing poorly at short term contracts. Pentanomial NIG models seem to outperform the other models, especially for long-dated maturities.

OBJECTIVES. The purpose of this study was to identify health-care seeking and related behaviors relevant to controlling sexually transmitted diseases in Kenya. METHODS. A total of 380 patients with sexually transmitted diseases (n = 189 men and 191 women) at eight public clinics were questioned about their health-care seeking and sexual behaviors. RESULTS. Women waited longer than men to attend study clinics and were more likely to continue to have sex while symptomatic. A large proportion of patients had sought treatment previously in both the public and private sectors without relief of symptoms, resulting in delays in presenting to study clinics. For women, being married and giving a recent history of selling sex were both independently associated with continuing to have sex while symptomatic. CONCLUSIONS. Reducing the transmission of sexually transmitted diseases in Kenya will require improved access, particularly for women, to effective health services, preferably at the point of first contact with the health system. It is also critical to encourage people to reduce sexual activity while symptomatic, seek treatment promptly, and increase condom use.

Multivariate data collected over time on the same experimental unit, referred to as multivariate longitudinal data, are typical of many agricultural, biological, clinical and medical studies. One way to account for the correlations that exist both within and across time is to express the variance-covariance matrix as the Kroneeker product of two matrices. These matrices, denoted by [Delta] and [Omega], reflect the characteristic and time dimensions underlying multivariate longitudinal data. The purpose of this thesis is to investigate the asymptotic relative efficiency (ARE) of hypothesis tests in the linear model for multivariate longitudinal data, evaluated through the trace asymptotic relative efficiency (TARE) and curvature asymptotic relative efficiency (CARE). The gain in efficiency from exploiting a Kronecker product covariance structure when it is appropriate is investigated. To estimate the TARE and CARE, a Monte-carlo simulation study is conducted. The loss of efficiency from imposing a Kronecker product model when it is not appropriate is also considered. Using a class of non-Kroneeker product covariance matrices and an index, which quantifies how far a given matrix departs from Kronecker product structure, a Monte-carlo simulation study is conducted. Ordinary least squares and generalised least squares procedures were also compared under a Kronecker product model. For the designs and covariance matrices considered, the gain in efficiency from exploiting the Kronecker product covariance structure is most pronounced when there is high correlation across time. For the class of non-Kronecker product covariance matrices defined, a noticeable loss of efficiency occurs when the covariance matrix is far from Kronecker product structure, in particular when there is a moderate departure from the null hypothesis under consideration. The use of ordinary least squares, which ignores cross-sectional and longitudinal correlations, is shown to be inefficient, especially when these correlations are high in absolute value

Morphological studies of the diagnostic characters of Amblyseius swirskii Athias-Henriot (original description) and A. enab El Badry (several new collections including the type locality) indicated close affinity between the two species. Among the Egyptian collections, little morphological variations have been observed except for setae Z1, S2, S4 and S5. Sequencing of a fragment of the internal transcribed spacer (ITS) region (ITS1, ITS2 and 5.8S) of different samples from Egypt showed almost complete identity with Neoseiulus swirskii (= A. swirskii Accession number EU 310505). Other related species i.e. A. cucumeris (Oud.), N. andersoni (Chant) and N. fallacis (Garman) showed distant identity. The intraspecific variationswere great between the six samples of Egypt and mainly because of mutations (which could be due to error during replication or as a result of mutagens).

Opijah FJ, Mukabana JR. "On the influence of urbanization on the water budget in Nairobi city: A numerical study.". 2004. AbstractOn the influence of urbanization on the water budget in Nairobi city: A numerical study

The impact of the growth and development of the City of Nairobi on the water budget is simulated using a high-resolution limited-area numerical model. The water substance fields are modelled with full physics in a control experiment. Five sensitivity experiments are then performed by altering the land-use/cover over the domain of study to assess the influence of the city, forests and terrain undulations on the water substance fields. Results showed that the highest evaporation occurred in areas of the study domain with open grasslands/scattered bush-land's vegetation types and the least at the city centre. Deforestation would lead to a substantial increase in the loss of water effected through evaporation despite a reduction in transpiration. The observed rainfall amount and frequency were highest in the high ground portions to the northwest of the study domain. Numerical analyses showed that the urban heat island had a destabilizing effect on the flow, which enhanced convection that resulted in increased rainfall downwind of the urban area. Further growth and expansion of the city of Nairobi would increase the area and amount of rainfall received. Deforestation would decrease rainfall amounts. Massive reforestation would increase the observed rainfall. There has been a decrease in soil moisture at the current location of the city centre; the decrease is bound to increase with the expansion of the city. The City of Nairobi has resulted in a large decrease in the soil moisture through converting the natural fabric to concrete/asphalt material. Deforestation would result in a marginal decrease in the soil moisture. Further growth and development of Nairobi City would modify the water substance budget appreciably.

This work highlights aspects of the R lmer function for a case where the dataset is nested, highly unbalanced, involves mixed effects and repeated measurements. The lmer function is part of the lme4 package of the statistical software R. The dataset used in the study is simulated from a survey of cow milk off takes from a group of Herds in Uganda, Africa. The purpose of the survey was to identify quality breeds of African Indigenous cattle for purposes of genetic breeding following the difficulties involved in implantation of foreign breeds of cattle in Africa. The work highlights the use of mixed model analysis in the context of animal breed selection. The exposition is accessible to readers with an intermediate background in statistics. Some previous exposure to R is helpful as well as some familiarity with mixed models.

Wireless sensor networks (WSNs) form a large part of the ecosystem of the Internet of Things (IoT), hence they have numerous application domains with varying performance and availability requirements. Limited resources that include processing capability, queue capacity, and available energy in addition to frequent node and link failures degrade the performance and availability of these networks. In an attempt to efficiently utilise the limited resources and to maintain the reliable network with efficient data transmission; it is common to select a clustering approach, where a cluster head is selected among the diverse IoT devices. This study presents the stochastic performance as well as the energy evaluation model for WSNs that have both node and link failures. The model developed considers an integrated performance and availability approach. Various duty cycling schemes within the medium-access control of the WSNs are also considered to incorporate the impact of sleeping/idle states that are presented using analytical modeling. The results presented using the proposed analytical models show the effects of factors such as failures, various queue capacities and system scalability. The analytical results presented are in very good agreement with simulation results and also present an important fact that the proposed models are very useful for identification of thresholds between WSN system characteristics.

In contrast to the hypothesis of Altshtein-Vogt dealing mainly with the problem of oncogene origin, the general theory of oncogenesis not only defines concretely the origin of the oncogene and the essence of its product, but also makes it possible to understand why, when and how integration of the oncogene with the genome of the cell leads to the transformation of the cell into a benign cell and when into a malignant tumour cell. An analysis of the essence of the "oncogene position effect" from this standpoint shows that an integration, similar in its mechanism but differing in polarity, of the genome of other viruses with the cell genome should lead to the formation of a corresponding antiviral stable (life-long) immunity or also to the emergence of pseudoautoimmune disease of the type caused by "slow" viruses.

Nathan I, Lund JF, Gausset Q, Andersen SK. "On the Promises of Devolution: Overcoming the Constraints of Natural Resource Management in a Village in Tanzania.". 2007. Abstract

This article is concerned with the hypothesis that devolution, understood as entrusting local government with significant domains of autonomous discretionary power, will lead to the equitable and efficient management of natural resources. The paper focuses on the three domains of power conceived by some theorists as critical in the management of natural resources, namely making rules, implementing rules, and resolving disputes in relation to these rules. Based on a case study of a village in Tanzania, the article identifies some of the main constraints the village council encounters concerning the efficient and equitable management of common lands, and discusses whether devolution is the solution for overcoming these constraints. It is concluded that the role and functions of higher levels of government in decentralised natural resource management are essential and require due consideration beyond the point of arguing for more autonomy to local government.

It is a well-known result in operator theory that whenever two operators are similar then they have equal spectra even though they do not have to belong to the same class of operators. However under a stronger relation of unitary equivalence it can be shown that two unitarily equivalent operators may belong to the same class of operators. In this paper we endeavor to exhibit results on such classes of operators which belong to same class under unitary equivalence.

Itis a known fact in operator theory that two similar operators have equal spectra but they do not necessarily have to belong to the same class of operators. However, under the stronger relation of unitary equivalence it can be shown that two unitarily equivalent operators may belong to the same class of operators. In this paper we endeavor to exhibit some results on some pairs of operators which may belong to the same class under not only unitary equivalence but also isometric and co-isometric equivalence

In this paper we investigate unitary quasi-equivalence of operators in Hilbert spaces. We characterize operators that are unitarily quasi-equivalent. We also investigate equivalence relations closely related to unitary quasi-equivalence. We give and prove conditions under which unitary quasi-equivalence coincides with other operator equivalence relations.

Increasing land pressure due to the rapidly growing population, growing demand for livestock products in urban centres and new land policies in Uganda are changing the life styles of the hitherto extensive grazers of the long horned Ankole cattle in south western Uganda. A production system where two separate herds are kept on one farm, a pure bred Ankole herd and a herd of Friesian - Ankole crosses is emerging. The Friesian-Ankole crosses are kept as a source of milk, most of which is sold to generate income. The Ankole are kept for cultural reasons, a buffer against shock in case of prolonged drought and disease outbreak and for income through sale of live animals. This study investigates the life cycle efficiency of the different genotypes on farm. Eighteen farmers have been selected and in each farm up to 30 animals have been selected per herd covering the complete age /sex range of the herd. During the selection the crossbred status was evaluated based on information from the farmer and phenotype. All selected animals were tagged. The animals are currently followed up on monthly intervals and this will last for a period of two years. During the visits body condition, tick count, health status, milk production of cows, growth parameters in young animals (weight approximated by the measurement of chest circumference) are recorded. Other information collected includes disease and parasite control measures (methods and costs involved), labour costs, calving dates, any supplementation, rainfall and temperature patterns. This paper discusses preliminary results on performance traits of the different genotypes for the first year of the study.

This integrative review on the teaching of reading in Kenyan primary schools provides a foundation for the growing movement there to improve reading education. In gathering sources for this review, we took an inclusive historical stance. Thus, we did not dismiss research reports that lacked traditional indicators of quality such as being published in peer-reviewed journals. We used multiple methods to find relevant research and associated documents, including two trips to Kenya. The review is organized by six topics: (a) language of instruction, (b) reading instruction, (c) reading materials, (d) reading culture, (e) assessment, and (f) teacher development. The review concludes with six proposals for policymakers, educational researchers, and teacher educators for the development of reading instruction based on what we learned in reviewing the literature. The first proposals are intended specifically to address the teaching of reading in Kenya, but they may be relevant to other sub-Saharan nations. The final proposal encourages others to conduct similar reviews to make possible a handbook of reading in Africa.

Bovine foscioliosis coused by F. giganticais widespread in   There is a large collection of reports of fasciolosis in Kenya based on  abattoir data records from veterinary investigation laboratories (VILS) as well as reports on a few farm study was carried out to improve on the reports.  Diagnosis of fasciola infection has traditionally been based on detection of typical eggs in the faeces.  A variety of other techniques are now available eg enzyme-linked immunosorbent assay (ELISA), which has shown to be sensitive and useful. Three agro-ecological zoned were defined depending on the reported prevalence; high risk, medium risk and low risk zones.  Two study districts were picked at random from each zone.  The study farms were selected using the two stage cluster sampling. Faecal and blood samples were collected on the farm.  Serum was later harvested.  ELISA and faecal sedimatation tests (FST) were carried out. A total of 2434 faecal and blood samples were screened.  ELISA achieved the highest (66%) positive rate of the samples from Kwale district and the lowest (23%) rate in Nakuru.  An overall positive prevalence of (43%) for fasciolosis was achieved.  The faecal sedimentation test showed prevalence of 19%.  In both tests high prevalence were observed in Kwale and Kilifi districts.  ELSA was always positive when FST was positive but not the converse. The on-famr survey utilizing two reliable diagnostic tests was meant to improve on existing abattoir reports.  Both tests showed fair to good agreements.  The higher detection by ELISA might be due to deworming and other reasons.  It was concluded that on-farm surveys are better than retrogressive studies; thought the latter are cheaper and faster.,  the current prevalence of fasciolosis are different from past reports with coastal showing higher than expected prevalences.

Bovine foscioliosls coused by F. gigantica is widespread in Africa, including Kenya. There is a large collection of reports on fasciolosis in Kenya based on abattoir data records from veterinary investigation laboratories (VILs) as well as reports on a few farm surveys. However, these reports are deficient in one way or another. Thus, a cross-sectional on-farm study was carried out to improve on the reports. Diagnosis of fasciola infection has traditionally been based on detection of typical eggs In the faeces. A variety of other techniques are now available e.g. enzyme-linked Immunosorbent assay '(ELlSA), which has shown to be sensitive and useful. Both resistance and susceptibility come with costs that may affect pre- and post-fertilization reproductive functions. We performed the study by crossing zucchini yellow mosaic virus resistant and susceptible pollen (from a hybrid donor) to infected and healthy maternal plants. We also analysed the pistil nutrient content in both treatments and found an increase of the susceptible and not resistant genotypes when maternal plants were infected. The level of nutrients K, P and S was higher in the pistils of the infected maternal plants than the healthy ones. Pistil nutrient level did not affect pollen tube growth rates, as indicated by seed siring patterns along the fruit. We propose that the effect on the siring ability of pollen carrying the susceptible and resistant alleles occurred at the post-fertilization stage, possibly as an indirect result of different growth rates of the two embryo genotypes under elevated nutrient conditions. We discuss our results with respect to possibilities of differential selection, costs and reproductive implications.

Mathari Hospital, 00100 Nairobi, Kenya. OBJECTIVES: To determine the prevalence rate of post traumatic stress disorder (PTSD) and associated risk factors among motor vehicle accident (MVA) survivors attending the orthopaedic and trauma clinic at Kenyatta National Hospital, Nairobi. DESIGN: A cross-sectional study. SETTING: Kenyatta National Hospital, Nairobi. SUBJECTS: One hundred and ninety seven adult males and sixty seven female patients. METHOD: The 264 patients were interviewed using a questionnaire to collect the socio-demographic data, the Self Rating Questionnaire (SRQ) and the Impact of Event Scale-Revised (IES-R). Diagnosis was made using the Diagnostic and Statistical Manual (DSM-IV). RESULTS: The mean age was 34.63+/-12.71 years (range 18-65). Overall, the prevalence rate of PTSD was 13.3%. None of the cases had been previously diagnosed as having PTSD. Females had a higher rate of 17.9% (n = 67), compared to the males 11.7% (n=197). The majority of those with PTSD (42.9%) were young, 20 - 29 years. Other risk factors were having post-primary education (62.9%), experiencing the first motor vehicle accident (14.1%), previous psychiatric illness, and other medical illnesses. The type of accident, role/status and immediate reactions to the accident were not significant. CONCLUSIONS: PTSD following motor vehicle accidents is common. Although the MVA survivors do develop significant rates of PTSD, it is not easy to identify those at risk but some of the parameters documented in this study may help. A multi disciplinary approach is therefore essential in the management of the RTA survivors at the orthopaedic and trauma clinics if their physical and psychological needs are to be adequately addressed PMID: 15490709 [PubMed - indexed for MEDLINE]

PIP: Malaria is the most prevalent and devastating public health problem in Africa despite much research and control effort over the last two decades. In most parts of Africa, individuals should take 200 mg of Proguanil daily together with chloroquine 5 mg/kg per week as prophylaxis. Pregnant women and individuals with underlying disease such as sickle cell making them susceptible to severe or complicated malaria, however, should take just 200 mg Proguanil daily. In hard-core multi-drug resistance areas, mefloquine 250 mg once weekly together with chloroquine 300 mg weekly is recommended as prophylaxis. Since no anti-malarial drug confers absolute protection against infection, however, using mosquito nets impregnated with permethrin, insecticides, and mosquito repellents is also advocated for those at high risk of severe malaria. The need also exists to treat cases of malaria when prevention is unsuccessful. Chloroquine in total dose 25 mg/Kg over three days is the first choice treatment of uncomplicated malaria in 4-aminoquinoline sensitive areas. Amodiaquine 25 mg/Kg over three days is the second line treatment, while pyrimethamine/sulphonamide combinations are useful in areas where there is resistance to 4-aminoquinalines. Finally, quinine 10 mg/kg every eight hours for seven days is the treatment of choice for severe and complicated malaria.

Unilateral variations in the formation of the median nerve, with the presence of the third head of the biceps brachii entrapping the nerve are very rare. These variations were observed on the right side, of a 30 year old male cadaver during routine dissection at the Department of Human Anatomy, University of Nairobi. The median nerve was formed by the union of three contributions; two from the lateral cord and one from the medial cord. An additional head of the biceps brachii looped over the formed median nerve. On the left side the median nerve was formed classically by single contributions from the medial and the lateral cords. These variations are clinically important because symptoms of high median nerve compression arising from similar formations are often confused with more common causes such as radiculopathy and carpal tunnel syndrome.

Background: Prolapsed intervertebral disk (PID) disease can be managed conservatively or surgically with different reported outcomes. Objective: The present study aimed at assessing the management and outcomes of slipped intervertebral disk disease at the Kenyatta National hospital.Study Design: A retrospective cross-sectional study.Setting: Kenyatta National Hospital (KNH) a referral and teaching Hospital in Kenya.Patients and Methods: Consecutive files of all cases of slipped intervertebral disk disease from January1997 to December 2007 were retrieved from the Medical records at the Kenyatta National Hospital. The biodata, management methods and the outcomes of the procedures were recorded. The collecteddata was analyzed using the SPSS 17.0 for Windows. Results: Six hundred and three cases were reviewed. All patients received analgesics and bed rest.Five percent of the patients were put on bilateral traction for two weeks while 4% of the patients had corsets. Thirty five per cent of the patients were surgically managed. Over a third of the surgically managed patients had laminectomies. Microdiscectomy was increasingly popular in the latter half ofthe study period. Of the managed patients 95% reported improvement while 92% were complication free. The rate of reherniation and reoperation was 1.5% and 1.2% respectively. Conclusion: The management of PID at Kenyatta National Hospital is largely successful with few cases of complications. In selected patients both conservative and surgical care are used in tandem. Microdiscectomy is an increasingly popular surgical procedure at the KNH.

Knowledge of the variant vascular anatomy of the subhepatic region is important for hepatobiliary surgeons in limiting operative complications due to unexpected bleeding. The pattern of arterial blood supply of 102 gallbladders was studied by gross dissection. The cystic artery originated from the right hepatic artery in 92.2% of cases. The rest were aberrant and originated from the proper hepatic artery. Accessory arteries were observed to originate from proper hepatic artery (n = 5), left hepatic artery (n = 2), and right hepatic artery (n = 1). Most of the arteries approached the gallbladder in relation to the common hepatic duct (anterior 45.1%, posterior, 46.1%). The other vessels passed anterior to common bile duct (2.9%), posterior to common bile duct (3.9%), or were given off in Calot's triangle. Cystic arteries in this data set show wide variations in terms of relationship to the duct systems. In about one tenth of patients, an accessory cystic artery may need to be ligated or clipped during cholecystectomy. 2007 Wiley-Liss, Inc

A study of malaria on the Kano Plain, Kisumu District, Western Kenya, was carried out between April and August, 1985. The study included a knowledge, attitudes and practices (K.A.P.) survey on malaria illness and the mosquito vector. Overall knowledge about malaria illness was found to be good. However, treatment and prevention practices of malaria were found to be poor. Knowledge of the mosquito and its relationship to malaria was found to be high. Knowledge of methods of prevention of mosquito bites was also found to be high but actual use of the methods was low. Knowledge of traditional methods of prevention of mosquito bites was also found to be high. Actual use was again found to be low.

OBJECTIVE: To determine the impact on neutrophils if adriamycin is administered at 60 mg/m2 and cyclophosphamide at 600/m2 (AC 60/600); and at 50 mg/m2 and 500 mg/m2 (50/500) in the treatment of breast cancer. DESIGN: Restrospective analysis of nadir neutrophil counts in female mammary carcinoma patients treated with adriamycin/cyclophosphamide combination. SETTING: Hurlingham Oncology Clinic, Nairobi and The Nairobi Hospital between March 1995 and August 1999. SUBJECTS: Eighteen patients with breast cancer were treated either for adjuvant purposes or for metastatic disease. INTERVENTION: Chemotherapy with adriamycin and cyclophosphamide at 60/600 or 50/500. Patients were advised to avoid crowded places and given prophylactic broadspectrum antibiotics whenever grade 4 neutropenia occurred at nadir. RESULTS: Grade 3-4 neutropenia occurred in 75.5% of treatments at 60/600 and in 56.8% of the treatments at 50/500. Febrile neutropenia followed only one treatment and did not result in death. CONCLUSION: Neutropenia is frequent and severe at A/C 60/600 and need to be watched out for. Sepsis on the other hand is prevented if meticulous attention is given and corrective measures taken. A/C 50/500 was associated with less occurrences of neutropenia though still very high. Neutropenia should therefore be checked and steps be taken to prevent sepsis even at this dosage.

Abstract A cross-sectional study was conducted to determine the prevalence, intensity and spectrum of helminths of free range pigs in Homabay District, Kenya. Faecal samples from 372 pigs were examined using the modified McMaster technique and post-mortem examination of 30 pigs carried out. Out of the 372 pigs examined, 308 (83%) were excreting nematode eggs. The nematode eggs encountered were those of Strongyles (75%), Strongyloides spp (26.6%), Trichuris spp (7.8%), Ascaris spp (5.4%) and Metastrongylus spp (0.3%). Coproculture of Strongyle-type nematode egg positive faecal samples revealed the presence of Oesophagostomum spp (74%), Hyostrongylus rubidus (22%) and Trichostrongylus spp (4%). The post-mortem examination revealed presence of Hyostrongylus rubidus, Physocephalus sexalatus, Trichostrongylus axei, Ascaris suum, Oesophagostomun dentatum, Trichuris suis and Metastrongylus pudendodectus. The highest prevalence of helminth infections was recorded in finishers (88%) and the lowest in adults (79%). The highest mean helminth egg per gram of faeces (epg) was recorded in adults (1,175) and the lowest was in piglets (526). Pigs from Riana division had the highest prevalence (91%) of infection and mean epg (1,109), while those from Asego Division had the lowest prevalence (50%) and mean epg (100). Female pigs recorded a higher mean epg (567) compared to males (416). Age had significant influence on infection with Strongyles (p = 0.04) with growers and finishers recording higher levels of infection than adults. Sex had significant effect on the prevalence of infections with Strongyles (p = 0.028) and Ascaris suum (p = 0.012) with females recording higher levels of infection than males. Division of origin of pigs had significant influence on the prevalence of infection with Ascaris suum (p = 0.000) and Strongyles (p = 0.000) with the mean epgs for Riana and Ndhiwa divisions being significantly higher than those of Pala Division. This study indicates that helminths are highly prevalent in the study area with low to moderate levels of infections and may be one of the contributing factors to low productivity. Therefore, there is need to formulate appropriate control measures for the parasites in order to increase livestock productivity.

Internet is the prototype of the global information infrastructure. Firms are using the Internet primarily for sales and service by advertising and providing customers with relevant information about a product or service. This paper describes the emerging implications of Internet technology as an evolving medium that offers restaurants limitless opportunities for e-commerce and for creating lasting relationships with customers. It dampens the bargaining power of channels by providing companies with new and more direct avenues to customers .The Internet provides an efficient means to order products. Online ordering being one of the e-commerce initiatives has emerged as an alternative avenue for food outlets to grow their sales. It offers more opportunities for interactive and personalised marketing as well as ease, speed and precision for the consumer. Food-outlets are able to offer the service through their own websites, multi-restaurant sites, mobile applications, and text ordering. Studies have shown that online food ordering can result in an increase in revenue, average check value, volume of sales, frequency of sales, order accuracy, productivity, improved convenience and customer relationship management. On line food ordering is new in Kenya. Few studies have been done in Kenya to determine the extent and distribution of online ordering by food outlets, and preference for distribution channels. From a population of 408 middle to upscale food outlets in Nairobi, this study sought answers to these questions. The results show that 23.7% of the outlets currently offer online ordering. The offerings were found to be independent of the restaurant type, but dependent on the type of cuisine offered. The characterization provided by this study lays the ground work for future work on the impact adoption has had on business performance.

Alcohol use is the 5th most important risk factor driving the global burden of diseases. WHO identifies a lack of health worker training as one of the main barriers to providing cost-effective brief interventions for alcohol use disorder. This study assesses the impact of online training, using the NextGenU.org model, on the delivery of the WHO Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) and its linked brief intervention (BI).

The pay-as-you-go pricing model and the illusion of unlimited resources in the Cloud initiate the idea to provision services elastically. Elastic provisioning of services allocates/de-allocates resources dynamically in response to the changes of the workload. It minimizes the service provisioning cost while maintaining the desired service level objectives (SLOs). Model-predictive control is often used in building such elasticity controllers that dynamically provision resources. However, they need to be trained, either online or offline, before making accurate scaling decisions. The training process involves tedious and significant amount of work as well as some expertise, especially when the model has many dimensions and the training granularity is fine, which is proved to be essential in order to build an accurate elasticity controller. In this paper, we present OnlineElastMan, which is a self-trained proactive elasticity manager for cloud-based storage services. It automatically evolves itself while serving the workload. Experiments using OnlineElastMan with Cassandra indicate that OnlineElastMan continuously improves its provision accuracy, i.e., minimizing provisioning cost and SLO violations, under various workload patterns.

We surveyed the phytoseid mites in four different geographical zones of Kenya: Zone I, upper highland and tropical alpine (2400-4400m): Zone II, lower highland (1800-2400m); zone III, midland (800-1800m); Zone IV, tropical, hot and humid( 0-800m ). A total of 107 species was found. In the sub family, amblyseeinae there were 14 species in the genus Neoseilus , one in Aspereroseius Chant, one in Paraphytoseius Swirski &Schechter, five in typhlodromips De Leon, five in Transeius Chant & McMurty, one in Graminaseius Chant &McMurty, 11 in Amblyseius Berlese, one in Arrenoseius Wanstein, two in Typhlodromalus muma, seven in Ueckemannseius Chant &McMurty, one in Ambylodromalus Chant &Mcmurty,, 20 in Euseius Wanstein, one in Iphiseius Berlese, one in Phytoseilus Evans and one in Gynaseius Ehara & Imano. In the subfamily Phytoseiinae Berlese there were four species in the genus Phytoseiius Ribaga. In the subfamily Typhlodrominae Wanstein, there were four species in the genus Kuzinellus Wainstein and 27 in Typhlodromus Scheuten
