This study reports concentrations of Mn, Fe, Cu and Zn analysed from Kenyan bean grains and leaves using Total Reflection X-ray Fluorescence (TXRF) and Inductively Coupled Plasma – Optical Emission Spectrometry (ICP-OES) methods. Samples of fresh bean leaves and 7 species of dry grains were collected from small scale farmers in Machakos and Kiambu counties. They were delivered to the laboratory where bean grain samples for TXRF analysis were prepared and leaves for both TXRF and ICP-OES. Standard methods of sample preparation successfully tested and used by other authors were used, Average elemental concentrations of the samples were then compared with established sufficiency values mainly given in ranges. Bean leaves from Muguga had sufficient Mn, Ni and Cu concentrations while 6% of the samples had Zn concentrations higher than the sufficiency range. Fe concentrations were consistently high with 97% of the samples having concentrations that were higher than the sufficiency range. No deficiencies were observed from bean leaves from Kyevaluki. For dry bean grains, deficiencies were observed in which four species were found to be deficient in Mn while Zn deficiency was observed in all the species. Fe concentrations were above the sufficiency ranges for all the species but not to toxic levels and 29% of the samples had Cu concentrations that were within the sufficiency range. The obtained data will be essential information for both farmers and policy makers on the nutritional quality of beans in the sampled areas. However, further studies are recommended including soil analysis and elemental bioavailability for full nutritional assessment. Background: Pneumonia is a major cause of morbidity
and mortality globally. Despite the proven benefits of
guideline concordant antibiotic prescribing, research
has shown that adherence to clinical guideline
recommendations is dismal.
Objectives: The study aims to determine utilization of
Kenyatta National Hospital antibiotic guideline titled
‘The KNH guide to empiric antimicrobial therapy
2018’ in the management of community acquired
pneumonia in the Kenyatta National Hospital medical
wards and the perceived barriers towards the utilization
of this guideline.
Materials and methods: A check list derived from the
Kenyatta National Hospital (KNH) guide to empiric
antimicrobial therapy 2018 was used to assess guideline
concordance based on seven quality indicators:
empiric antibiotic, dose and route of administration,
switch to oral antibiotics, duration of antibiotics (at
least 5 days), collection of microbiological samples
before initiating antibiotics, review of antibiotics at 48
hours and once the culture results are out. Online selfadministered questionnaires were used to determine
attitude and perceived barriers towards utilization
of the KNH guideline among the Internal Medicine
registrars and medical officers.
Analysis: Descriptive statistics were applied in
the representation of each of the seven quality
indicators. These were then compared with the
guideline recommendations and adherence to the
guideline for each parameter was expressed as a
percentage of the total number of patients admitted
with community acquired pneumonia. These were
then graded into the following categories based on the
level of concordance: Good >90%, Intermediate 60-
90%, poor <60%. Questions on the attitude and the
perceived barriers towards KNH guideline utilization
were answered using a 5 point Likert scale. Perceived
barrier statements that were positively formulated
were then recorded so that a lower score meant a
lower level of the perceived barriers and vice versa.
Percentages were then calculated for the total number
of doctors that agreed or strongly agreed that the
barrier was applicable. An open ended question on the
top three barriers to the KNH guideline utilization was
also included in the questionnaire.
Results: For each of the other quality indicators,
adherence to the KNH guideline for patients with
community acquired pneumonia was as follows:
empiric antibiotic choice 48%, collection of samples
for culture prior to antibiotic administration 0%, review
of antibiotics at 48 hours 26.4%, review of antibiotics
with culture results 45.8%, total duration of antibiotics
28.8% and time to switch to oral antibiotics 3.6%. The
top three barriers towards guideline utilization among
the doctors were: unavailability of drugs (52.7%),
inaccessibility of the KNH guideline (45.1%) and lack
of or delay of investigations (34.1%).
Conclusion: This study has demonstrated that the level
of adherence to the seven quality indicators from the
KNH guide is poor with the overall adherence being
35.5%. The recommendation least adhered to was
collection of microbiological samples before initiation
of empiric antibiotics. The most commonly identified
barriers to utilization of the guideline were external
and guideline related barriers.

The data revolution has increased data demands for leveraging on Big Data in the production of statistics. The paper assesses the adoption of Big Data in research institutes in Kenya. Data were collected from 64 data practitioners based in the 24 research institutes that have a mandate to produce and analyse official statistics. The paper establishes the risks and challenges of using Big Data in statistics, identifies the determinants of adoption of Big Data in statistics and validates the relevance of a Technology Adoption Model (TAM) for predicting the adoption. It is the conclusion that there are immense opportunities for Big Data in statistics if the associated risks and challenges are addressed and the identified key determinants prioritized to promote the adoption.

The boron-doped diamond electrode has been widely applied in electrochemical process for wastewater treatment based on the generation of hydroxyl radicals (•OH). This study investigated the formation efficiency of 2-HTA through the interaction between •OH radical and terephthalic acid (TA) to monitor the •OH formation. Results show that the 2-HTA formation efficiency did not depend on the initial concentration of scavenger (here, TA) in the investigated range of 0.1 to 1 mM TA. Meanwhile, other factors, such as current density, supporting electrolyte, sodium hydroxide have a significant influence. The optimal condition was established: concentration of electrolyte (0.05 M Na2SO4); initial concentration of TA (0.1 mM); concentration of NaOH (1 mM); current density (j = 20 mA cm–2). The selectivity of TA was evaluated by Faradaic efficiency (η) and hydroxylation yield (γTAOH), indicating that Faradaic efficiency (η) for 2-HTA formation was in the range of 10–5–10–7, while the hydroxylation yield of •OH (γTAOH) into 2-HTA was 0.03–0.12.

Background: Rheumatoid Arthritis
(RA) affects 0.5-1% of the adult
population. A higher prevalence of
thyroid dysfunction is observed in
patients with RA compared to the
general population.
Objectives: To establish the
frequency of thyroid dysfunction
among ambulatory RA patients and
to describe the association between
thyroid dysfunction and the patients’
socio-demographic characteristics,
clinical characteristics, level of disease
activity, and their functional status.
Design: This was a cross-sectional
descriptive study.
Methods: Adult patients on follow
up for RA at the outpatient clinic were
sampled. Sociodemographic data was
recorded. The Clinical Disease Activity
Index (CDAI) and Health Assessment
Questionnaire (HAQ) scores were
computed from examination findings
and questionnaires respectively. A
venous blood sample was analyzed for
Thyroid-Stimulating Hormone (TSH),
free triidothyronine (fT3), and free
tetraiodothyronine (fT4). This data
was analyzed to determine frequencies
and associations.
Results: Seventy-six patients were
recruited into the study. Sixty-one
participants were female. The mean
TSH level was 5.8Miu/L. The frequency
of thyroid dysfunction was 47.4%.
Overt hypothyroidism was the most
common form of thyroid dysfunction at
39.5% while 6.6% had Sick Euthyroid.
Majority of the participants, 75%, had
low disease activity, mean CDAI was
11.6. Forty-one (53.9%) participants
had no disability, mean HAQ was
0.5. Correlations between thyroid
dysfunction and advancing age,
duration of disease, level of disease
activity, and functional disability did
not attain statistical significance.
Conclusion: Thyroid dysfunction
is common among patients with RA
with no significant association found
between thyroid dysfunction sociodemographic characteristics, clinical
characteristics, level of disease activity,
and functional status.

Introduction
Rheumatoid Arthritis (RA) is a
symmetric polyarthritis with a variety
of systemic manifestations. In the
general population thyroid dysfunction
affects 1-10% of adults, with variations
in geographical areas, age and sex1
.
The causes of thyroid dysfunction
include; iodine deficiency, infections
and autoimmune associated thyroid
disease2
. Thyroid dysfunction is more
prevalent in patients with autoimmune
diseases such as RA. This is attributed
to overlap of autoimmune conditions
that are initiated by loss of tolerance to
self-antigens3
.
The burden of thyroid dysfunction
among RA patients has been found
to vary between 6-47% in various
studies. The entire spectrum of thyroid
dysfunction has been described,
however, hypothyroidism occurs
more frequently. Patients with thyroid

activity scores and poorer functional

Background: The oral cavity harbors many microbes that may cause diseases, including dental caries and periodontal diseases. Progressive inflammation from periodontal diseases may lead to gum detachment from the teeth. Povidone-iodine and chlorhexidine mouth rinses and gargles are broad-spectrum antimicrobial products that effectively manage dental caries and periodontal diseases and eliminate plaques. This study was conducted in Nairobi County, Kenya to establish the quality of povidone-iodine and chlorhexidine oral care products by determining the content of the active pharmaceutical ingredient and compliance with labeling requirements.
Methods: A total of 34 samples (from 15 brands) of povidone-iodine and 15 samples (from nine brands) of chlorhexidine were collected from retail pharmacies using convenience sampling. All samples were subjected to labeling analysis, identity, and assay tests. Potentiometric titration was used to assay povidone-iodine in the samples, while chlorhexidine was assayed using high-performance liquid chromatography (HPLC) according to British Pharmacopeia 2017 specifications.
Results: All samples complied with identification tests. Moreover, 47.1% of povidone-iodine and 66.7% of chlorhexidine products complied with pharmacopoeial assay specifications. Five povidone-iodine (14.7%) and four chlorhexidine (26.7%) samples had missing label information on the storage conditions and the address of the manufacturer.
Conclusions: Strict adherence to current Good Manufacturing Practices (cGMP) by manufacturers of povidone-iodine and chlorhexidine mouthwashes/gargles is necessary to guarantee quality assured products in the market. Regular post-market surveillance and regulatory enforcement of standards are instrumental in minimizing the circulation of poor-quality products.
