ODHIAMBO DRALFRED. "Comparison of ct and MRI in various pathoanatomic regions of the body.". Abstract

The birth of radiology in 1895 was accidental. However its place in disease investigation is sealed. The ground breaking discovery invited awe and indignation in almost equal measure. The medical operative of the day was quick to recognize the potential it possessed in the investigation of medical ailments. To the less technically endowed, the development represented an assault on human dignity and privacy. No human mortal they argued was entitled to know what lay beyond the skin. To them the discovery was a bad omen.
Fast forward to 1971 and computed tomography was to explode on the medical scene. This discovery considered by many the most important discovery since the establishment of X rays allowed the evaluation of the 3rd dimension in human anatomy. In coming decades the technology would evolve immeasurably. Today computed tomography finds relevant applications in imaging of virtually all body regions.
So robust was CT in its diversity of uses that the protracted development of MRI technology caught manufacturers and doctors alike napping. In any event Sir Peter Hounsfield stumbled on MRI as he attempted to find a suitable replacement to X ray radiodiagnosis. To this end his efforts came to a cropper. His persistence and that of coworkers was to unleash the incomparable imaging tool that it has become..
Today the twin technologies of CT and MRI straddle the imaging field like a colossus. The pace of innovation is mind boggling. Even imaging specialists find it challenging to remain up to date on more than one front of diagnostic radiology. The dilemma of the clinician in being able to identify the most cost effective imaging study then becomes obvious.
The most important question for them is to ask whether regional biases exist. Are there pathoanatomic regions in which one tool holds sway? Can a simple “cook book” be developed to guide the doctor who has the patient in his surgery on the most appropriate imaging approach? Knowing the general ball park diagnostic avenue would profoundly reduce the number of supporting investigations apart the study of choice lowering costs to the patient. Waiting lists would shorten as would idiosyncrasies.

ODHIAMBO DRALFRED, BYAKIKA DRBESSIE, GACHIE DRANDREW, OLIECH PROFJOSEPH. "Lower urinary tract symptoms (LUTS) as the initial presentation for colorectal malignancy.". Abstractluts_presentation.pdf

The presenting features of colorectal carcinoma in the African patient are not different from reports worldwide although the majority present late with features of advanced disease (1). Increasing frequency of diagnosis may be attributed the twin effects of changing diets and easier access particularly by the elite population to modern diagnostic imaging tools.
A 72 year old African male presented to a private practitioner with a 2 week history hematuria, frequency, nocturia and subsequently a 2 day history of retention of urine (AUR). Imaging studies demonstrated a tumour mass invading the roof of the urinary bladder. Following surgical extirpation the mass was subjected to histopathological evaluation.

A cholinomimetic substance was isolated from desiccated venom of Dendroaspis polylepis by one-dimensional ascending paper chromatography. The migratory and staining properties of the substance were compared with those of standard acetylcholine. Pharmacological and biochemical identification was carried out on various in vitro and in vivo biological test objects as well as with high-voltage paper electrophoresis. The assay of the cholinomimetic substance was done on both superfused guinea pig ileum and hamster stomach strips. The cholinomimetic substance content was 2.44 - 3.46mg/0.96gm of total protein in the desiccated venom.

Cerebral venous sinus thrombosis is an uncommon yet potentially life threatening condition which is often under-diagnosed or misdiagnosed because of the variability of the clinical signs and symptoms. The imaging findings are often subtle thus the need for a high index of suspicion. The superior sagittal and transverse sinuses are the most frequently involved. The correct diagnosis of cerebral venous thrombosis relies on neuroimaging studies. The two cases reported highlight the fact that the radiologist may be the first clinician to suspect and diagnose this condition. This ultimately affects prognosis as early diagnosis and treatment reduces morbidity and mortality.

In: Soil Tillage for Crop Production and Protection of the Environment. Proceedings of 13th Inte.". In: Bloemfontein, South Africa. Kisipan, M.L.; 1194. Abstract
This paper reviews some research studies on tillage methods influencing soil and moisture conservation in the eastern African countries of Kenya, Tanzania, Malawi and Ethiopia during the past four decades. Most of these studies were conducted in marginal rainfall (semi arid ) areas and on shallow soils of various textures (sandy clay loam, sandy clay, clay and loam). The studies were meant to establish the effects of tillage and residue management practices on physico-chemical soil properties (i.e. structure, bulk density, soil moisture and organic matter contents), runoff and infiltration. This review emphasizes the importance of appropriate tillage and residue management methods (contour bunds and terraces, minimum tillage, tied ridging, mulching and conventional tillage) in providing soil conditions favourable for soil moisture conservation and subsequent crop performance and yield on smallholder farms.

A sample of 166 normal adult males and females was taken from different colleges of Assiut representing those living in upper Egypt. Each subject has been studied for measurements of stature (S), hand length (HL) and hand breadth (HB). The data were statistically analysed in order to assess the relationship between stature and hand measurements. The correlation matrix of the study indicates close similarity of the relationship between stature and hand measurements in both sexes and in both sides. A generalized multiple regression equation has been designed to estimate stature from values of hand length and hand breadth regardless of sex or side in the form: S = 34.5 + 5.77 HL + 2.7 HB +/- 5.1. This equation may be helpful to obtain approximate stature when there is difficulty in obtaining a direct measurement or where there is a chance print of a criminal or an amputated hand or arm.

Fusarium wilt caused by Fusarium udum Butler, is an economically important disease of pigeonpea (Cajanus cajan (L.) Mill). Information on the mechanisms of resistance to this disease in pigeonpea is limited. To study the role of vascular occlusion in wilt resistance, isolates of F.udum were inoculated onto resistant and susceptible varieties of pigeonpea and observed under light and transmission electron microscopes. The presence of F. udum in wilt susceptible plants was characterized by mycelia and conidia in the xylem vessels, plugging in some vessels, disintegration of xylem parenchyma cells in the infected areas, and the formation of cavities due to heavy colonization in the pith cortex vascular bundle. Resistance to F. udum in the roots and stems of wilt resistant plants was associated with low fungal colonization and high occlusion due to tyloses and gels in the xylem vessels. There were significant differences (P = 0:05) in the number of xylem vessels occluded by tyloses in resistant and susceptible plants with a maximum of 22.5% and 8.0% occlusion, respectively. It is probable that tyloses and gels formed as a result of F. udum interaction in wilt resistant plants are part of a resistance mechanism. Key words: Fusarium wilt, cajanus cajan, resistance, tylose, vascular occlusion

In a five year retrospective study of 360 patients with homozygous (SS) sickle cell disease, eighteen (5%) were found to have neurological complications. Their ages ranged from 7 months to 21 years with a mean of 11.1 +/- 6 years. Of those with neurological complications, twelve (67%) of the patients had cerebrovascular accident, six (33.3%) convulsions, three visual disturbance; one sensorineural deafness, one cerebellar degeneration and the last one confusion and hallucinations. Four of the patients had multiple neurological complications. There was only one patient with recurrence of neurological complications. Two patients were hypertransfused and up to the end of the study period none of them had any recurrence. The pattern of neurological complications are similar to that observed in other studies. However, in this study, there were fewer recurrences of neurological complications.

As the number of fatalities from cardiovascular diseases declines in western industrial nations, an opposite trend is observed in the East African region. Inter-regional variations in the prevalence of vascular disorders have been attributed to socioeconomic, psychosocial and heritable physiological parameters. Although faulty mineralocorticoid metabolism and the dysfunctional kidney are prominent features of circulatory problems, many current studies are focused on membrane receptors, transmembrane ion transport mechanisms, ion channels and the possible genetic polymorphisms that determine the characteristics of those molecular structures in the vascular system of normal or hypertensive persons. In this review, a composite of the data available on each of the above parameters and its significance in the pathogenesis of hypertension in the industrial West and transforming economies of East Africa is presented.

The human body louse, Pediculus humanus, showed eighteen midgut proteins ranging between 12 and 117 kDa, when analysed by SDS-PAGE electrophoresis. Seven of them (12 kDa, 17 kDa, 29 kDa, 35 kDa, 40 kDa, 55 kDa and 97 kDa) were major bands based on their intensity of staining. The immunization of rabbits with a midgut extract elicited the production of protective polyclonal antibodies. These antibodies reacted strongly with all major midgut proteins as well as with 63 kDa and 117 kDa proteins when tested by the Western blot technique. The analysis of the proteins revealed that the 12 kDa, 25 kDa, 29 kDa, 35 kDa, 45 kDa, 87 kDa and 97 kDa proteins are glycosylated and none of them contained a lipid moiety. By electroelution, the proteins of 35 kDa and 63 kDa were purified. On trypsinization, the proteins of 35 kDa and 63 kDa produced four major fragments (F1, F2, F3, and F4) when resolved on a 18% SDS-PAGE. The F1 fragment of the 35 kDa protein reacted with the polyclonal antibodies by the immunoblot technique.

Serum acid phosphatase was measured in patients with enlarged benign and malignant prostate before and after rectal examination. Amongst the patients with benign glands, rectal examination did not produce any significant false elevation of the enzyme. Rectal examination, however, caused a rise in the enzyme level in a few untreated cancer patients and in cancer patients who has become refractory to hormonal therapy. This rise would help rather than mislead in the diagnosis of malignant prostate and also in the identifying treated patients who had become refractory to treatment. Thus, when serum acid phosphatase is properly determined, elevated levels should always arouse suspicion of malignant prostate or other lesions associated with high enzyme level even is such determination was preceded by rectal examination. There appears to be no merit in the teaching that the determination of serum acid phosphatase should be delayed after rectal examination.

This paper investigates the relationships between knowledge of common sexually transmitted diseases and or HIV/AIDS and sexual behaviour change among adolescents in school and out of school in Nairobi City. Despite the existence of a well-structured health education system supported by informal health educators, it is apparent that health education knowledge acquired is not commensurate with desired changes in attitudes and behaviour. This study sampled 250 adolescents in Nairobi City aged between 14 to 24 years. Furthermore, focus group discussions were conducted to supplement views of individual respondents. The respondents socio-economic and cultural backgrounds represented well the major societal groups and classes. It was found that 93.2 percent of adolescents were knowledgeable of sexually transmitted diseases and over 90 per cent accurately identified symptoms of common sexually transmitted diseases and HIV/AIDS. However, it also emerged that peer pressure and peer networks have a very strong impact on attitude and behaviour of adolescent. The study recommends that more research is needed to understand how effects of peer networks could be made more effective in reducing adolescents pre-marital sex.

The thickness of the visual cortex, the diameter of cell nuclei and the mean volume of internuclear material per nucleus in the visual cortex, geniculate bodies and superior colliculi were determined in growing and adult mice. The influence of prolonged stay in complete darkness was investigated. In the visual cortex of normal mice, a peak in nuclear size occurred 20–30 days after birth, followed by a peak in relative volume of internuclear material. In growing mice, reared in darkness from birth, a highly significant decrease in relative volume of internuclear material occurred in all visual centers. The decrease was greatest in the geniculate bodies and greater in the granular and supragranular layers of the cortex than in the infragranular. In the cortex, the decrease was most pronounced after two months and became less pronounced during prolonged stay in darkness. No similar normalization was observed in subcortical visual centers. In the same mice, visual deprivation caused a highly significant decrease in the thickness of the visual cortex and in the diameter of its cell nuclei. The decrease in nuclear diameter was greater in the granular and supragranular than in the infragranular layers of the cortex and exhibited a similar normalization during prolonged stay in darkness as the decrease in relative volume of internuclear material. In adult mice, visual deprivation caused decrease in internuclear material.

Differentiation of bloodstream-form trypanosomes into procyclic (midgut) forms is an important first step in the establishment of an infection within the tsetse fly. This complex process is mediated by a wide variety of factors, including those associated with the vector itself, the trypanosomes and the bloodmeal. As part of an on-going project in our laboratory, we recently isolated and characterized a bloodmeal-induced molecule with both lectin and trypsin activities from midguts of the tsetse fly, Glossina longipennis [Osir, E.O., Abubakar, L., Imbuga, M.O., 1995. Purification and characterization of a midgut lectin-trypsin complex from the tsetse fly, Glossina longipennis. Parasitol. Res. 81, 276-281]. The protein (lectin-trypsin complex) was found to be capable of stimulating differentiation of bloodstream trypanosomes in vitro. Using polyclonal antibodies to the complex, we screened a G. fuscipes fuscipes cDNA midgut expression library and identified a putative proteolytic lectin gene. The cDNA encodes a putative mature polypeptide with 274 amino acids (designated Glossina proteolytic lectin, Gpl). The deduced amino acid sequence includes a hydrophobic signal peptide and a highly conserved N-terminal sequence motif. The typical features of serine protease trypsin family of proteins found in the sequence include the His/Asp/Ser active site triad with the conserved residues surrounding it, three pairs of cysteine residues for disulfide bridges and an aspartate residue at the specificity pocket. Expression of the gene in a bacterial expression system yielded a protein (M(r) approximately 32,500). The recombinant protein (Gpl) bound d(+) glucosamine and agglutinated bloodstream-form trypanosomes and rabbit red blood cells. In addition, the protein was found to be capable of inducing transformation of bloodstream-form trypanosomes into procyclic forms in vitro. Antibodies raised against the recombinant protein showed cross-reactivity with the alpha subunit of the lectin-trypsin complex. These results support our earlier hypothesis that this molecule is involved in the establishment of trypanosome infections in tsetse flies.

Hematologic values for peripheral blood and marrow, serum proteins, B12 and folic acid levels are given for recently caught and captive baboons (P. anubis and P.sinocephalus) in Nairobi. The hemaglobin levels were higher in captive than in wild animals. There was no change in the serum proteins unless the animals were on a riboflavindeficient diet. Marrow activity as estimated by counting red cell precursors as a percentage of the total nucleated red cell population agreed with that of red cell uptake of Fe59 ferric chloride. Hemosiderin was present in the marrow in all the animals. Protein turnover studies using I131-labeled albumin with ion exchange resins indicated that the protein catabolic rate and turnover was high. This may have been due to the selective destruction of the human albumin in the baboon.

A clinical, radiological, histological, and geographical study of carcinoma of the esophagus in Kenya is reported. It was found that this cancer is diagnosed more often in males than in females, with a ration of 8:1 (Kenya Cancer Registry). The most common age in males and females is 50 to 59 years. The regions of the esophagus most commonly involved are the middle and lower thirds, in almost equal proportions. An unexplained tendency for the tumor to be poorly differentiated towards the lower third is noted. In addition a review on the possible etiological factors is presented.

A study that devised a modified method of reporting antibiotic sensitivity results was undertaken. Enterobacteriaceae and Gram positive cocci were tested for drug sensitivity by a disc diffusion method. Zones of bacterial growth inhibition were measured, dividing the isolates into four groups: the highly sensitive, the moderately sensitive, the slightly sensitive and the resistant ones. The slightly sensitive isolates were taken as indicators of antibiotic resistance acquisition. By that system, when more than 50% of the isolates fell into the slightly and resistant groups, that meant that the antibiotic concerned would be discontinued for some time until the bacteria reverted to being moderately sensitive. The study also provided a method of making antibiotic discs from local blotting papers, and a sample of a form on which antibiotic sensitivity results could be recorded was presented. The method is considered to be easy and very appropriate for developing countries in detecting gradual and abrupt acquisition of antibiotic resistance by bacteria.

There appears to be no merit in the teaching that the determination of serum acid phosphatase should be delayed after rectal examination.

Serum acid phosphatase was measured in patients with enlarged benign and malignant prostate before and after rectal examination. Amongst the patients with benign glands, rectal examination did not produce any significant false elevation of the enzyme.

There is a marked shortage of surgical manpower all over Africa. General surgeons undertake a very wide range of surgical work because of the lack of specialists. Orthopaedic and general surgeons both care for accident injuries. Current training and recruitment programs are inadequate in correcting existing gross manpower deficiencies. The situation is further aggravated by a gross maldistribution of available manpower in favor of large urban centers. In many parts of rural Africa, minor surgical procedures are carried out by suitably trained, non-physician health workers, but facilities and resources for surgery outside urban centers are generally inadequate. The World Health Organization program on essential surgical and anesthetic procedures, which is currently under way in collaboration with the International Federation of Surgical Colleges, the World Federation of Societies of Anesthesiologists, and other professional bodies, should have a significant impact on these urgent needs for basic surgery in rural district hospitals.

The effect of visual deprivation on synaptic contacts in the visual cortex was studied with the electron microscope. The deprivation was achieved by unilateral lid suture in 14-day old white rats before eye opening after which the animals were kept for 8 weeks. The density and the size of synaptic contacts in the upper part of the visual cortex (from the surface of layer II up to V) were estimated. The mean density of synapses of the visual cortex supplied by the deprived eye was 20% less than on the control side, the upper levels of the cortex being more affected. The mean size of synaptic contacts was larger (+7.5%) in the upper levels of the deprived cortex. No right left difference in density and size were found in control animals.

This integrative review on the teaching of reading in Kenyan primary schools provides a foundation for the growing movement there to improve reading education. In gathering sources for this review, we took an inclusive historical stance. Thus, we did not dismiss research reports that lacked traditional indicators of quality such as being published in peer-reviewed journals. We used multiple methods to find relevant research and associated documents, including two trips to Kenya. The review is organized by six topics: (a) language of instruction, (b) reading instruction, (c) reading materials, (d) reading culture, (e) assessment, and (f) teacher development. The review concludes with six proposals for policymakers, educational researchers, and teacher educators for the development of reading instruction based on what we learned in reviewing the literature. The first proposals are intended specifically to address the teaching of reading in Kenya, but they may be relevant to other sub-Saharan nations. The final proposal encourages others to conduct similar reviews to make possible a handbook of reading in Africa.

Objective: To determine the prevalence and pattern of eye diseases and visual impairment in the Nairobi Comprehensive Eye Care Services (NCES) Project; the catchment area of the Mbagathi District Eye Unit of Nairobi. Design: Community based survey conducted from October 15th to 31st 2007 Setting: Kibera and Dagoreti divisions of Nairobi City Subjects: 4200 people of all ages were randomly selected; 4056 were examined (96.6% response rate). 122 (2.9%) were not available and 15 (0.4%) declined to be examined. Results: Females: 54.2%, Males: 45.8%. Mean age; 22.4 years, SD; 16.5. Only 241(5.9%) aged >50years old. The leading eye disorders in Kibera and Dagoretti divisions are conjunctival disorders including allergic conjunctivitis and conjunctival growths. This was found to affect 7.6% of the subjects. This was followed by refractive errors found in 5.3% of the subjects. Cataract was found in 30 subjects (0.7%). Disorders of the retina and the optic nerve were found in 1.1% of the subjects and corneal disorders in 0.5%. The prevalence of visual impairment was 0.6%, severe visual impairment was 0.05% and blindness was 0.1%. This indicates that most of the ocular disorders encountered were not visually threatening. The main cause of visual impairment is refractive errors and the causes of severe visual impairment and blindness are cataract, corneal opacity and glaucoma. Conclusion: The population of the NCES is relatively young and the prevalence of blindness and visual impairment is low. The main cause of visual impairment was refractive errors and the causes of severe visual impairment and blindness were cataract, corneal opacity and glaucoma. Recommendations: The level of blindness in NCES is low and the project should focus more on rendering eye care and not treatment of blindness. There is need to address the issue of refractive errors as this was one of the main ocular problems encountered. In this survey, it was not possible to perform detailed refraction and hence it was recommend that a refractive error survey be conducted; especially in school going children.

The gene Q13L coding for the Capripoxvirus group specific structural protein P32 was expressed in Escherichia coli using plasmid pGEX-2T as a fusion protein with glutathione-s-transferase and purified on glutathione sepharose affinity chromatography column. The protein was then employed for diagnosis of sheeppox, goatpox and lumpyskin disease, by a latex agglutination test (LAT) using the purified P32 antigen and guinea pig detector antiserum raised against the P32 antigen. The LAT and virus neutralization test (VNT) were used to screen one hundred livestock field sera for antibodies to Capripoxvirus, in comparison the LAT was simpler, rapid and 23% more sensitive than the VNT. In addition the LAT was found to be specific for Carpripoxvirus because it did not pick antibodies to Orthopoxvirus and Parapoxvirus. The LA test can be taken for a simple and quick diagnostic tool for primary screening of Carpripoxvirus infection and will reduce the reliance of diagnostic laboratories on tissue culture facilities. In addition the research went ahead to establish the avenues available to mobile phone manufacturers and users in order to enhance safe mobile disposal through a survey.  The research revealed that there was the need for the mobile manufacturers to manufacture handsets from recyclable materials. The manufacturers should also avail information on handset disposal at the point of sale and do a follow-up using the available media. The mobile services providers can also play an important role on mobile phone disposal by availing the information on the face of the scratch cards. In addition they can notify the subscribers on the available disposal channels through text messages. The government through the designated agencies such as the Communications Commission of Kenya (CCK) and National Environmental Management Authority (NEMA) needs to be more assertive in mobile phone disposal measures.
