Why do many smallholder farmers fail to adopt improved land management practices which can improve yields and incomes? The reason is not always because these practices are uneconomical but sometimes it is because resource poverty prevents farmers from taking advantage of yield and income enhancing agricultural practices. In this study we examine the relative merits of using a carbon payment scheme compared to a subsidy policy to help reduce the cost of specific land management practices with productivity and ecosystem benefits such as carbon sequestration. Using a 30-year crop simulation model, we examine the impacts of different soil fertility management treatments (SFTs) on yields and soil carbon and proceed to compute discounted incremental revenue streams over the same period. We find that the SFTs simulated are on average profitable given the conditions assumed in our DSSAT simulations. When carbon was priced at $8 or $12/t CO2e, the increase in incremental incomes generated from a carbon payment were invariably higher than those imputed from a 50% fertilizer subsidy. When carbon was priced at $4/Co2e, the increase was almost similar and sometimes higher than that from the imputed income transfer from a 50% subsidy. If these indications hold in further research, it could imply that using fertilizer subsidies as the sole mechanism for stimulating adoption of improved soil fertility management practices may unnecessarily forgo other complementary and possibly superior alternatives. Depending on the specific economic equity considerations, we conclude that either of these instruments can be used to help farmers break through resource barriers that prevent them from adopting productivity-enhancing and environmentally beneficial agricultural practices. However, given the fiscal burden on public finances and possible opportunity costs of any substantial subsidy program, it is possible that a carbon payment system can be a reasonable alternative assuming the range of carbon prices used in this study and especially if accompanied by measures to ameliorate the costs of fertilizer to farmers.

We surveyed the phytoseid mites in four different geographical zones of Kenya: Zone I, upper highland and tropical alpine (2400-4400m): Zone II, lower highland (1800-2400m); zone III, midland (800-1800m); Zone IV, tropical, hot and humid( 0-800m ). A total of 107 species was found. Background/aim: Glaucoma accounts for 8% of global blindness and surgery remains an important treatment. We aimed to determine the impact of adding simulation-based surgical education for glaucoma.

Methods: We designed a randomised controlled, parallel-group trial. Those assessing outcomes were masked to group assignment. Fifty-one trainee ophthalmologists from six university training institutions in sub-Saharan Africa were enrolled by inclusion criteria of having performed no surgical trabeculectomies and were randomised. Those randomised to the control group received no placebo intervention, but received the training intervention after the initial 12-month follow-up period. The intervention was an intense simulation-based surgical training course over 1 week. The primary outcome measure was overall simulation surgical competency at 3 months.

Results: Twenty-five were assigned to the intervention group and 26 to the control group, with 2 dropouts from the intervention group. Forty-nine were included in the final intention-to-treat analysis. Surgical competence at baseline was comparable between the arms. This increased to 30.4 (76.1%) and 9.8 (24.4%) for the intervention and the control group, respectively, 3 months after the training intervention for the intervention group, a difference of 20.6 points (95% CI 18.3 to 22.9, p<0.001). At 1 year, the mean surgical competency score of the intervention arm participants was 28.6 (71.5%), compared with 11.6 (29.0%) for the control (difference 17.0, 95% CI 14.8 to 19.4, p<0.001).

Conclusion: These results support the pursuit of financial, advocacy and research investments to establish simulation surgery training units and courses including instruction, feedback, deliberate practice and reflection with outcome measurement to enable trainee glaucoma surgeons to engage in intense simulation training for glaucoma surgery.

BackgroundManagement and control of the COVID-19 pandemic caused by the severe acute respiratory syndrome coronavirus SARS-CoV-2 is critically dependent on quick and reliable identification of the virus in clinical specimens. Detection of viral RNA by a colorimetric reverse transcription loop-mediated isothermal amplification (RT-LAMP) is a simple, reliable and cost-effective assay, deployable in resource-limited settings (RLS). Our objective was to evaluate the intrinsic and extrinsic performances of RT-LAMP in RLS.
Methods
This is a multicenter prospective observational study of diagnostic accuracy, conducted from October 2020 to February 2021 in four African Countries: Cameroon, Ethiopia, Kenya and Nigeria; and in Italy. We enroled 1657 individuals who were either COVID-19 suspect cases, or asymptomatic and presented for screening. RNA extracted from pharyngeal swabs was tested in parallel by a colorimetric RT-LAMP and by a standard real time polymerase chain reaction (RT-PCR).
Findings
The sensitivity and specificity of index RT LAMP compared to standard RT-PCR on 1657 prospective specimens from infected individuals was determined. For a subset of 1292 specimens, which underwent exactly the same procedures in different countries, we obtained very high specificity (98%) and positive predictive value (PPV = 99%), while the sensitivity was 87%, with a negative predictive value NPV = 70%, Stratification of RT-PCR data showed superior sensitivity achieved with an RT-PCR cycle threshold (Ct) below 35 (97%), which decreased to 60% above 35.
Interpretation
In this field trial, RT-LAMP appears to be a reliable assay, comparable to RT-PCR, particularly with medium-high viral loads (Ct < 35). Hence, RT-LAMP can be deployed in RLS for timely management and prevention of COVID-19, without compromising the quality of output.

In this study, the antileishmanial and cytotoxic activities of secondary metabolites isolated from Tabernaemontana ventricosa Hochst. ex A. DC., Aloe tororoana Reynolds, and Aloe schweinfurthii var. labworana Reynolds were investigated. Overall, nineteen known compounds were isolated from the three plant species. The compounds were characterized based on their spectroscopic data. Voacristine and aloenin were the most active compounds against promastigotes of antimony-sensitive Leishmania donovani (IC50 11 ± 5.2 μM and 26 ± 6.5 µM, respectively) with low toxicity against RAW264.7, murine monocyte/macrophage-like cells. The in silico docking evaluation and in vitro NO generation assay also substantially support the antileishmanial effects of these compounds. Background: Africa has one of the highest incidences of gonorrhea. Neisseria gonorrhoeae is gaining resistance to most of the available antibiotics, compromising treatment across the world. Whole-genome sequencing (WGS) is an efficient way of predicting AMR determinants and their spread in the population. Recent advances in next-generation sequencing technologies like Oxford Nanopore Technology (ONT) have helped in the generation of longer reads of DNA in a shorter duration with lower cost. Increasing accuracy of base-calling algorithms, high throughput, error-correction strategies, and ease of using the mobile sequencer MinION in remote areas lead to its adoption for routine microbial genome sequencing. To investigate whether MinION-only sequencing is sufficient for WGS and downstream analysis in resource-limited settings, we sequenced the genomes of 14 suspected N. gonorrhoeae isolates from Nairobi, Kenya.

Methods: Using WGS, the isolates were confirmed to be cases of N. gonorrhoeae (n = 9), and there were three co-occurrences of N. gonorrhoeae with Moraxella osloensis and N. meningitidis (n = 2). N. meningitidis has been implicated in sexually transmitted infections in recent years. The near-complete N. gonorrhoeae genomes (n = 10) were analyzed further for mutations/factors causing AMR using an in-house database of mutations curated from the literature.

Results: We observe that ciprofloxacin resistance is associated with multiple mutations in both gyrA and parC. Mutations conferring tetracycline (rpsJ) and sulfonamide (folP) resistance and plasmids encoding beta-lactamase were seen in all the strains, and tet(M)-containing plasmids were identified in nine strains. Phylogenetic analysis clustered the 10 isolates into clades containing previously sequenced genomes from Kenya and countries across the world. Based on homology modeling of AMR targets, we see that the mutations in GyrA and ParC disrupt the hydrogen bonding with quinolone drugs and mutations in FolP may affect interaction with the antibiotic.

Conclusion: Here, we demonstrate the utility of mobile DNA sequencing technology in producing a consensus genome for sequence typing and detection of genetic determinants of AMR. The workflow followed in the study, including AMR mutation dataset creation and the genome identification, assembly, and analysis, can be used for any clinical isolate. Further studies are required to determine the utility of real-time sequencing in outbreak investigations, diagnosis, and management of infections, especially in resource-limited settings.

Description
Five known compounds (1–5) were isolated from the extract of Mundulea sericea leaves. Similar investigation of the roots of this plant afforded an additional three known compounds (6–8). The structures were elucidated using NMR spectroscopic and mass spectrometric analyses. The absolute configuration of 1 was established using ECD spectroscopy. In an antiplasmodial activity assay, compound 1 showed good activity with an IC50 of 2.0 μM against chloroquine-resistant W2, and 6.6 μM against the chloroquine-sensitive 3D7 strains of Plasmodium falciparum. Some of the compounds were also tested for antileishmanial activity. Dehydrolupinifolinol (2) and sericetin (5) were active against drug-sensitive Leishmania donovani (MHOM/IN/83/AG83) with IC50 values of 9.0 and 5.0 μM, respectively. population acquiring arms illegally for their protection. The study sought to explore the key challenges facing AMISOM in its bid to
restore peace in Somalia. The study was based on the theory of conflict transformation. The study utilized primary data, which was
collected using structured questionnaires and interview schedules. The unit of analysis constituted a sample size of 100 respondents
drawn from the military, police, and civilian components of AMISOM and Somali civilian population working within Halane Base
Camp, with a combined target population of 24,643. Quantitative data was descriptively analyzed using the percentages and qualitative
data was analyzed using themes emerging from the data. The study found out thatclan dynamics in the conflict torn Somalia remains
the biggest hindrance to the achievement of peace and stability. This is coupled by the weak government structures that underpin
inadequacy of support to AMISOM operations. The study recommends that there isneed for AMISOM to increase involvement with
civilians as they stand important in peace building initiatives, intelligence, mediation and prevention of conflict.

Description
The first phytochemical investigation of the flowers of Millettia dura resulted in the isolation of seven isoflavones, a flavonol and a chalcone. Eleven isoflavones and a flavonol isolated from various plant parts from this plant were tested for cytotoxicity against a panel of cell lines, and six of these showed good activity with IC50 values of 6-14 μM. Durmillone was the most active with IC50 values of 6.6 μM against A549 adenocarcinomic human alveolar basal epithelial cancer cell line with low cytotoxicity against the non-cancerous cell lines BEAS-2B (IC50 = 58.4 μM), LO2 hepatocytes (IC50 78.7 μM) and CCD19Lu fibroblasts (IC50 >100 μM).

Abstract
Keloids are fibroproliferative disorders characterized by high recurrence rates, with few factors known to influence the same. We conducted a study to determine whether keloid histology influences recurrence. This was a prospective longitudinal study to determine whether histopathological parameters of keloid influence recurrence. Patients with keloids managed by surgical excision were followed up at Kenyatta National Hospital between August 2018 and July 2020. The excised keloids were processed for histology using hematoxylin,/eosin, Masson, and trichrome stains. The slides were analyzed for inflammatory cells, fibroblasts, and capillary density using the hot spot technique and correlated to keloid recurrence. Postoperative follow-up was for a minimum of 1 year. A total of 90 patients with 104 keloids were recruited in the study. Overall keloid recurrence rate was 28.6%. There was a correlation between the absolute count of more than 50 per High power field of lymphocytes, fibroblasts, and macrophages with recurrence of the disease. The sensitivity and specificity for the above parameters were lymphocytes 48% and 81%, macrophages 57% and 83%, mast cells 32% and 33%, and fibroblasts 41% and 91%, respectively. There was no correlation between mast cells and vascularity status with recurrence. Routine histology should, therefore, be performed to determine these parameters. Close monitoring and second-line therapy should be considered for patients with elevated macrophages and/or lymphocytes so as to reduce the risk of recurrence.

Universal health coverage (UHC) is an integral part of the United Nations sustainable development goals. The private sector plays a prominent role in achieving UHC, being the primary source of essential medicines for many people. However, many private healthcare facilities in low‐ and middle‐income countries (LMICs) have insu_cient stocks of essential medicines. At the same time, these same facilities carry excessive quantities of certain drugs, leading to obsolescence. This suggests poor inventory control. To propose potential remedies it is vital to fully understand the underlying causes. In semi‐structured interviews with managers of private healthcare facilities in Nairobi, we asked them about their 1) inventory control systems, 2) inventory control skills, 3) time/human resource constraints, 4) budget constraints, 5) motivations for inventory control, and 6) suppliers. Our results suggest that the problems are driven by resource limitations (budget and time/human resources), managerial issues (relating to skills and systems), and market mechanisms that limit overage and underage costs. Unavailability at the supplier level and motivations for inventory control are relatively minor issues. We posit that the key causes are interlinked and stem from wider issues in the market and regulatory environment. Our results challenge prevalent beliefs about medicine supply chains in LMICs and lead to alternative hypotheses. Testing these hypotheses could improve our understanding of inventory management in private healthcare facilities and aid progress in achieving UHC.

Objectives. To identify the changes in rheumatology service delivery across the five regions of Africa from the
impact of the COVID-19 pandemic.
Methods. The COVID-19 African Rheumatology Study Group created an online survey consisting of 40 questions
relating to the current practices and experiences of rheumatologists across Africa. The CHERRIES checklist for
reporting results of internet e-surveys was adhered to.
Results. A total of 554 completed responses were received from 20 countries, which include six in Northern
Africa, six in West Africa, four in Southern Africa, three in East Africa and one in Central Africa. Consultant grade
rheumatologists constituted 436 (78.7%) of respondents with a mean of 14.56 10.3 years of experience. A total of
77 (13.9%) rheumatologists avoided starting a new biologic. Face-to-face clinics with the use of some personal
protective equipment continued to be held in only 293 (52.9%) rheumatologists’ practices. Teleconsultation modalities
found usage as follows: telephone in 335 (60.5%), WhatsApp in 241 (43.5%), emails in 90 (16.3%) and video
calls in 53 (9.6%). Physical examinations were mostly reduced in 295 (53.3%) or done with personal protective
equipment in 128 (23.1%) practices. Only 316 (57.0%) reported that the national rheumatology society in their
country had produced any recommendation around COVID-19 while only 73 (13.2%) confirmed the availability of a
national rheumatology COVID-19 registry in their country.
Conclusion. COVID-19 has shifted daily rheumatology practices across Africa to more virtual consultations and
regional disparities are more apparent in the availability of local protocols and registries.

Importance: Cataracts account for 40% of cases of blindness globally, with surgery the only treatment.

Objective: To determine whether adding simulation-based cataract surgical training to conventional training results in improved acquisition of surgical skills among trainees.

Design, setting, and participants: A multicenter, investigator-masked, parallel-group, randomized clinical educational-intervention trial was conducted at 5 university hospital training institutions in Kenya, Tanzania, Uganda, and Zimbabwe from October 1, 2017, to September 30, 2019, with a follow-up of 15 months. Fifty-two trainee ophthalmologists were assessed for eligibility (required no prior cataract surgery as primary surgeon); 50 were recruited and randomized. Those assessing outcomes of surgical competency were masked to group assignment. Analysis was performed on an intention-to-treat basis.

Interventions: The intervention group received a 5-day simulation-based cataract surgical training course, in addition to standard surgical training. The control group received standard training only, without a placebo intervention; however, those in the control group received the intervention training after the initial 12-month follow-up period.

Main outcomes and measures: The primary outcome measure was overall surgical competency at 3 months, which was assessed with a validated competency assessment rubric. Secondary outcomes included surgical competence at 1 year and quantity and outcomes (including visual acuity and posterior capsule rupture) of cataract surgical procedures performed during a 1-year period.

Results: Among the 50 participants (26 women [52.0%]; mean [SD] age, 32.3 [4.6] years), 25 were randomized to the intervention group, and 25 were randomized to the control group, with 1 dropout. Forty-nine participants were included in the final intention-to-treat analysis. Baseline characteristics were balanced. The participants in the intervention group had higher scores at 3 months compared with the participants in the control group, after adjusting for baseline assessment rubric score. The participants in the intervention group were estimated to have scores 16.6 points (out of 40) higher (95% CI, 14.4-18.7; P < .001) at 3 months than the participants in the control group. The participants in the intervention group performed a mean of 21.5 cataract surgical procedures in the year after the training, while the participants in the control group performed a mean of 8.5 cataract surgical procedures (mean difference, 13.0; 95% CI, 3.9-22.2; P < .001). Posterior capsule rupture rates (an important complication) were 7.8% (42 of 537) for the intervention group and 26.6% (54 of 203) for the control group (difference, 18.8%; 95% CI, 12.3%-25.3%; P < .001).

Conclusions and relevance: This randomized clinical trial provides evidence that intense simulation-based cataract surgical education facilitates the rapid acquisition of surgical competence and maximizes patient safety.

A major risk factor for the spread of livestock diseases and their vectors is the uncontrolled transboundary movement of live animals for trade and grazing. Such movements constrain effective control of tick-transmitted pathogens, including Theileria parva. Only limited studies have been undertaken to identify ticks and tick-borne diseases (TTBDs) affecting cattle in central African countries, including Cameroon. We hereby report the collection of baseline data on the prevalence of T. parva in Cameroon through a countrywide cross-sectional survey, conducted in 2016, involving collection of blood samples from cattle from 63 sites across the five agro-ecological zones (AEZs) of the country. ELISA-based surveillance of infected cattle was performed on 479 randomly selected samples and revealed specific antibodies to T. parva in 22.7% and T. mutans in 41.1% of cattle. Screening of 1,340 representative DNA samples for the presence of T. parva identified 25 (1.86%) positives using a p104 antigen gene-based nested PCR assay. The positives were distributed across agro-ecological zones I, II, III and V. None of the p104 positive cattle exhibited clinical symptoms of East Coast fever (ECF). Using reverse line blot (RLB), 58 (4.3%) and 1,139 (85%) of the samples reacted with the T. parva and T. mutans oligonucleotide probes, respectively. This represents the first report of T. parva from Cameroon. Surprisingly, no Rhipicephalus appendiculatus ticks, the main vector of T. parva, were identified in a parallel study involving comprehensive morphological and molecular survey of tick species present in the country. Only two of the 25 p104 positive cattle were PCR-positive for the CD8+ T-cell target schizont-expressed antigen gene Tp1. Cloning and sequencing of Tp1 amplicons revealed sequence identity with the reference T. parva Muguga. This new finding raises serious concerns of a potential spread of ECF into the central African region.

Treatment of gonorrhea is complicated by the development of antimicrobial resistance in Neisseria gonorrhoeae (GC) to the antibiotics recommended for treatment. Knowledge on types of plasmids and the antibiotic resistance genes they harbor is useful in monitoring the emergence and spread of bacterial antibiotic resistance. Abstract
Phytochemical analysis of a methanol–dichloromethane (1:1) extract of the aerial parts of Tephrosialinearis led to the isolation of 18 compounds. Seven of these, namely, lineaflavones A–D (1–4), 6-methoxygeraldone (5), 8″-acetylobovatin (6), and 5-hydroxy-7-methoxysaniculamin A (7) are new compounds. The compounds were characterized based on their NMR and HRMSn data. The anti-inflammatory effects of the crude extract and isolated compounds were evaluated by measuring the levels of interleukins (IL-1β, IL-2, and IL-6), granulocyte-macrophage colony-stimulating factor (GM-CSF), and tumor necrosis factor-α (TNF-α) in lipopolysaccharide (LPS)-stimulated peripheral blood mononuclear cells (PBMCs). The crude extract inhibited the release of all cytokines except IL-1β, which slightly increased in comparison to the LPS control. All the tested compounds suppressed the production of IL-2, GM-CSF, and TNF-α. Whereas compounds 1, 2, 4–8, 10–15, 17, and 18 decreased production of IL-6, compounds 1, 2, 4, 7, 10, 13–15, and 17 inhibited the release of IL-1β. It is worth noting that most of the compounds tested showed a superior reduction in cytokines release compared to the reference drug ibuprofen.

In an antiplasmodial activity assay, compound 1 showed good activity with an IC50 of 2.0 μM against chloroquine-resistant W2, and 6.6 μM against the chloroquine-sensitive 3D7 strains of Plasmodium falciparum. Some of the compounds were also tested for antileishmanial activity. Dehydrolupinifolinol (2) and sericetin (5) were active against drug-sensitive Leishmania donovani (MHOM/IN/83/AG83) with IC50 values of 9.0 and 5.0 μM, respectively. In a cytotoxicity assay, lupinifolin (3) showed significant activity on BEAS-2B (IC50 4.9 μM) and HePG2 (IC50 10.8 μM) human cell lines. The structures of the isolated compounds were elucidated by NMR and MS analyses. MTT assay was used to measure cell viability. Using in silico docking, the interaction of the compounds with key target proteins in the p53 pathway was determined. Results: From the root bark of this plant five compounds were isolated, namely; dihydrochelerythrine (1), skimmianine (2), tridecan-2-one (3), sesamin (4) and hesperidin (5). Dihydrochelerythrine (1) inhibited proliferation of liver cancer (HCC) cells (IC50 21.2), breast cancer (BT549) cells,(IC50 21.2 μM). Similarly, sesamin (4) exhibited moderate inhibitory activity against BT549 cancer cells (IC50 47.6 μM). Abstract

Introduction: Fifteen million babies are born prematurely, before 37 weeks gestational age, globally. More than 80% of these are in sub-Saharan Africa and Asia. 35% of all deaths in the first month of life are due to prematurity and the neonatal mortality rate is eight times higher in low-income and middle-income countries (LMICs) than in Europe. Early Warning Scores (EWS) are a way of recording vital signs using standardised charts to easily identify adverse clinical signs and escalate care appropriately. A range of EWS have been developed for neonates, though none in LMICs. This paper reports the findings of early work to examine if the use of EWS is feasible in LMICs.

Methods: We conducted an observational study to understand current practices for monitoring of preterm infants at a large national referral hospital in Nairobi, Kenya. Using hospital records, data were collected over an 8-week period in 2019 on all live born infants born at <37 weeks and/or <2500 g (n=294, 255 mothers) in the first week of life. Using a chart adopted from the EWS developed by the British Association of Perinatal Medicine, we plotted infants' vital signs. In addition, we held group discussions with stakeholders in Kenya to examine opinions on use of EWS.

Results: Recording of vital signs was variable; only 63% of infants had at least one temperature recorded and 53% had at least one heart rate and respiratory rate recorded. Stakeholders liked the traffic-light system and simplicity of the chart, though recognised challenges, such as staffing levels and ability to print in colour, to its adoption.

Conclusion: EWS may standardise documentation and identify infants who are at higher risk of an adverse outcome. However, human and non-human resource issues would need to be explored further before development of an EWS for LMICs.

Three distinct aquifers, which we collectively refer to as the Lodwar Alluvial Aquifer System, underlie Lodwar and its environs, the shallow alluvial, intermediate, and deep aquifers which are the main source of fresh water. A fourth, the shallow aquifer of the Turkana grit, is highly saline and with fluoride contamination. Just as the Turkwel River, the shallow alluvial aquifer (SAA) was dominated by Ca–HCO3 water type, while the TGSA was Na–Cl water type and became Na–HCO3 near the Holocene sediments. The intermediate aquifer (IA) was Na–HCO3water type. Pockets of Mg–HCO3 water occurred in the shallow alluvial and intermediate aquifers. The natural processes in the SAA include rock-water interaction, recharge by surface water, and oxidation reactions, while evaporation and dissolution are the major factors controlling the chemistry of the TGSA. Ion exchange, dilution, and dissolution are the major processes in the IA. Elevated levels of NO3− and SO42− during the wet season within the SAA and the IA reflects their vulnerability to pollution. Background
Globally, 15 million infants are born preterm each year, and 1 million die due to complications of prematurity. Over 60% of preterm births occur in Sub-Saharan Africa and south Asia. Care at birth for premature infants may be critical for survival and long term outcome. We conducted a prospective audit to assess whether women giving birth preterm could be identified, and to describe cord clamping and neonatal care at hospitals in Africa and south Asia.

Methods
This prospective audit of livebirths was conducted at six hospitals in Uganda, Kenya, India and Pakistan. Births were considered preterm if between 28+ 0 and 33+ 6 weeks gestation and/or the birthweight was 1.00 to 1.99 kg. A pre-specified audit plan was agreed with each hospital. Livebirths before 28 weeks gestation with birthweight less than 1.0 kg were excluded. Data were collected on estimated and actual gestation and birthweight, cord clamping, and neonatal care.

Results
Of 4149 women who gave birth during the audit, data were available for 3687 (90%). As 107 were multiple births, 3781 livebirths were included, of which 257 (7%) were preterm. Antenatal assessment correctly identified 148 infants as ‘preterm’ and 3429 as ‘term’, giving a positive predictive value of 72% and negative predictive value of 97%. For term births, cord clamping was usually later at the two Ugandan hospitals, median time to clamping 50 and 76 s, compared with 23 at Kenyatta (Kenya), 7 at CMC (India) and 12 at FBH/LNH (Pakistan). At the latter two, timing was similar between term and preterm births, and between vaginal and Caesarean births. For all the hospitals, the cord was clamped quickly at Caesarean births, with Mbale (Uganda) having the highest median time to clamping (15 s ‘term’, 19 ‘preterm’). For preterm infants temperature on admission to the neonatal unit was below 35.5 °C for 50%, and 59 (23%) died before hospital discharge.

Conclusions
Antenatal identification of preterm birth was good. Timing of cord clamping varied between hospitals, although at each there was no difference between ‘term’ and ‘preterm’ births. For premature infants hypothermia was common, and mortality before hospital discharge was high.

PURPOSE: This study compares a web-based teleophthalmology assessment with a clinical slit lamp examination to screen for diabetic retinopathy (DR) and age-related macular degeneration (AMD) among diabetic patients in a rural East African district.
METHODS: Six hundred and twelve eyes from 306 diabetic patients underwent both a clinical slit lamp examination and a teleretina (TR) assessment by an experienced ophthalmologist. Both assessments were compared for any DR and AMD using the early treatment diabetic retinopathy study and age-related eye disease study grading scales, respectively.
RESULTS: Of the 612 TR assessment photos, 74 (12%) were deemed ungradable due to media opacities, poor patient cooperation, or unsatisfactory photographs. The ability to detect DR and AMD showed a fair agreement (kappa statistic 0.27 and 0.23, respectively) between the TR and clinical slit lamp examination. Relative to a clinical slit lamp evaluation, a positive TR diagnosis carried a 75.0% positive predictive value when diagnosing DR and a 27.3% positive predictive value when diagnosing AMD. A negative TR diagnosis carried a 97.2% negative predictive value for the diagnosis of DR and a 98.1% negative predictive value for the diagnosis of AMD.
CONCLUSION: When comparing TR assessments to clinical slit lamp examinations to diagnose DR and AMD, there was a fair agreement. Although further validation is needed, the TR approach provides a promising method to diagnose DR and AMD, two major causes of ocular impairment worldwide.

Dengue fever (DF) is an arboviral disease caused by dengue virus serotypes 1-4 (DENV 1-4). Globally, DF incidence and disease burden have increased in the recent past. Initially implicated in a 1982 outbreak, DENV-2 recently reemerged in Kenya causing outbreaks between 2011 and 2014 and more recently 2017-8. The origin and the evolutionary patterns that may explain the epidemiological expansion and increasing impact of DENV-2 in Kenya remain poorly understood. Using whole-genome sequencing, samples collected during the 2011-4 and 2017-8 dengue outbreaks were analyzed. Additional DENV-2 genomes were downloaded and pooled together with the fourteen genomes generated in this study. Bioinformatic methods were used to analyze phylogenetic relationships and evolutionary patterns of DENV-2 causing outbreaks in Kenya. The findings from this study have shown the first evidence of circulation of two different Cosmopolitan genotype lineages of DENV-2; Cosmopolitan-I (C-I) and Cosmopolitan-II (C-II), in Kenya. Our results put the origin location of C-I lineage in India in 2011, and C-II lineage in Burkina Faso between 1979 and 2013. C-I lineage was the most isolated during recent outbreaks, thus showing the contribution of this newly emerged strain to the increased DENV epidemics in the region. Our findings, backed by evidence of recent local epidemics that have been associated with C-I in Kenya and C-II in Burkina Faso, add to the growing evidence of expanding circulation and the impact of multiple strains of DENV in the region as well as globally. Thus, continued surveillance efforts on DENV activity and its evolutionary trends in the region, would contribute toward effective control and the current vaccine development efforts.

Five new compounds—rhodimer (1), rhodiflavan A (2), rhodiflavan B (3), rhodiflavan C (4), and rhodacarpin (5)—along with 16 known secondary metabolites, were isolated from the CH2Cl2–CH3OH (1:1) extract of the roots of Tephrosia rhodesica. They were identified by NMR spectroscopic, mass spectrometric, X-ray crystallographic, and ECD spectroscopic analyses. The crude extract and the isolated compounds 2–5, 9, 15, and 21 showed activity (100% at 10 μg and IC50 = 5–15 μM) against the chloroquine-sensitive (3D7) strain of Plasmodium falciparum.

Access to water and sanitation remain a challenge in many developing countries, especially in pro-poor
urban informal settlements where socioeconomic livelihoods are generally low. The aim of this study was to characterise the water and sanitation facilities in the informal settlements of Kisumu City and to evaluate their effect on
community hygiene and health. The study focussed on the five urban informal settlements of Nyalenda A, Nyalenda
B, Manyatta A, Manyatta B and Obunga, and the three Peri-urban informal settlements of Kogony, Usoma and
Otonglo. Using descriptive techniques, the researcher surveyed 114 water sources and all sanitation facilities within
0-15m and 15-30m radii of the water sources. The findings revealed dominance of shallow wells and traditional pit
latrines as the primary water sources and sanitation facilities, respectively. Out of the water sources studied, 87.7%
(100) were shallow wells (mean depth 1.5 m), 9.6% (11) springs and 2.6% (3) boreholes. Most of these shallow
wells (83%) were within the urban informal settlements where uses range from washing and cleaning, cooking, and
even drinking (13.5%), despite the majority being unprotected. The analysis of the density of sanitation facilities
near the water points showed that 32.3% existed within a 15m radius of the nearest water sources, in violation of
the recommended safe distance of 30m. With an increased density of toilets near critical water sources and other
sanitary practices, public health is highly compromised.

Groundwater is the preferred alternative water source during times of shortages and in areas not served by piped water supplies. Pit latrines are the main sanitation facilities in Kisumu where sewerage extends over less than 20 per cent of the city. Pit latrines contribute to microbial contamination of shallow groundwater in Kisumu.

A set of structurally related O-methylated flavonoid natural products isolated from Senecio roseiflorus (1), Polygonum senegalense (2 and 3), Bhaphia macrocalyx (4), Gardenia ternifolia (5), and Psiadia punctulata (6) plant species were characterized for their interaction with human monoamine oxidases (MAO-A and-B) in vitro. Compounds 1, 2, and 5 showed selective inhibition of MAO-A, while 4 and 6 showed selective inhibition of MAO-B. Compound 3 showed~ 2-fold selectivity towards inhibition of MAO-A. Binding of compounds 1–3 and 5 with MAO-A, and compounds 3 and 6 with MAO-B was reversible and not time-independent. The analysis of enzyme-inhibition kinetics suggested a reversible-competitive mechanism for inhibition of MAO-A by 1 and 3, while a partially-reversible mixed-type inhibition by 5. Similarly, enzyme inhibition-kinetics analysis with compounds 3, 4, and 6, suggested a competitive reversible inhibition of MAO-B. The molecular docking study suggested that 1 selectively interacts with the active-site of human MAO-A near N5 of FAD. The calculated binding free energies of the O-methylated flavonoids (1 and 4–6) and chalcones (2 and 3) to MAO-A matched closely with the trend in the experimental IC 50′ s. Analysis of the binding free-energies suggested better interaction of 4 and 6 with MAO-B than with MAO-A. Investigations of the root extract of Dracaena usambarensis Engl. for anticancer principles led to the characterization of one new homoisoflavonoid, (3S)-3,4ʹ,5,6-tetrahydroxy-7-methoxyhomoisoflavanone (1) and a new retrodihydrochalcone, 4ʹ,4-dihydroxy-2,3-dimethoxydihydrochalcone (2) along with six previously reported compounds, including two homoisoflavonoids, 7-O-methyl-8-demethoxy-3-hydroxy-3,9-dihydropunctatin (3) and loureiriol (4); a phenolic amide, 3-(4ʹʹʹ-hydroxyphenyl)-N-[2ʹ-(4ʹʹ-hydroxyphenyl)-2ʹ-methoxyethyl]acrylamide (5); a spirostane, 25S-spirosta-1,4-dien-3-one (6) and two steroids, stigmasterol (7) and stigmasterol 3-O-β-D-glucopyranoside (8). The structures of 1-8 were determined using spectroscopic and spectrometric techniques. The absolute configurations of compounds 1 and 3 were achieved using circular dichroism spectroscopy. Using the resazurin reduction assay and doxorubicin as reference anticancer drug, 1 showed moderate cytotoxicity against drug sensitive CCRF-CEM but was inactive against all the other tested drug sensitive, resistance phenotypes and normal cells. The crude extract and 2-8 were inactive in the preliminary screening against CCRF-CEM and drug resistant CEM/ADR5000 cell lines. Interestingly, the activity of the standard drug, doxorubicin was comparable to those of inactive compounds against CEM/ADR5000 cells. Future studies should focus on structure modifications of 1-3, in order to obtain more potent analogues.

Introduction
The World Health Organization’s (WHO) Labour Care Guide (LCG) is a “next‐generation” partograph based on WHO’s latest intrapartum care recommendations. It aims to optimize clinical care provided to women and their experience of care. We evaluated the LCG’s usability, feasibility, and acceptability among maternity care practitioners in clinical settings.

Methods
Mixed‐methods evaluation with doctors, midwives, and nurses in 12 health facilities across Argentina, India, Kenya, Malawi, Nigeria, and Tanzania. Purposively sampled and trained practitioners applied the LCG in low‐risk women during labor and rated experiences, satisfaction, and usability. Practitioners were invited to focus group discussions (FGDs) to share experiences and perceptions of the LCG, which were subjected to framework analysis.

Results
One hundred and thirty‐six practitioners applied the LCG in managing labor and birth of 1,226 low‐risk women. The majority of women had a spontaneous vaginal birth (91.6%); two cases of intrapartum stillbirths (1.63 per 1000 births) occurred. Practitioner satisfaction with the LCG was high, and median usability score was 67.5%. Practitioners described the LCG as supporting precise and meticulous monitoring during labor, encouraging critical thinking in labor management, and improving the provision of woman‐centered care.

Conclusions
The LCG is feasible and acceptable to use across different clinical settings and can promote woman‐centered care, though some design improvements would benefit usability. Implementing the LCG needs to be accompanied by training and supportive supervision, and strategies to promote an enabling environment (including updated policies on supportive care interventions, and ensuring essential equipment is available).

Ensuring water security to urban population in fragile environments through interlinked systems of groundwater abstraction, storage and distribution of sufficient quantity is challenging especially to urban utilities situated in arid and semi-arid regions. The purpose of this research was to evaluate water delivery challenges for water utilities in fragile environment in Kenya. A systematic analysis of availability from each supply sub-components from source to consumer was carried out through water audit and network analysis by employing water flow measurement equipments and through pump performance analysis and by employing continuity equation and Bernoulli’s principle to sections of the network. Results showed that water availability within a utility in such environments is contributed by seasonal variations between wet and dry affecting quantity at source, optimal design of supply infrastructure
in this case better matching of solar power with the pump, using standard pipes and on optimal operational strategies employed to reduce losses within the network. Based on these findings, we conclude that with clear understanding of each subcomponent’s contributions to entire water supply system and optimizing their design and operations, more people will be made water secure in all seasons in the fragile environments.

Geogenic controls on groundwater quality are often dominant but remain inadequately characterized for many aquifer systems. The volcano-sedimentary aquifer of Kisumu (Kenya) is used widely to provide safe water to the informal settlements and acts as a strategic, supplementary supply to the city during interruptions in service from the main piped water supply network drawing from Lake Victoria. Little is known, however, of the geogenic controls on groundwater quality in the Kisumu aquifer. We characterize the origin and composition of solutes in groundwaters sampled from the Kisumu aquifer using a range of techniques. Classical graphical methods (i.e. Durov, Piper, Schoeller, Stiff and Ternary plots) were used as interpretative tools of the main hydrogeochemical processes whereas principal component analysis (PCA) and hierarchical cluster analysis (HCA) methods were used to assess hydrochemical variations and water types. An agglomeration schedule with five cluster solutions and between-groups linkage method of clustering using the squared Euclidian distance was employed. Variables were standardized to z-scores so that each variable contributed equally to the clusters. The study identifies three main groundwater geochemical signatures in the Kisumu study area: cation exchange (Ca-Na, Ca-Mg) between aqueous and solid phases, the chemistry of recharge water, and groundwater mixing. The concentration of major ions in groundwater varies with geology and also seasonally. The dominant water facies is Na-Ca-HCO3 type; other hydrochemical facies include Ca-Mg-HCO3 and Na-HCO3. Hydrochemical plots suggest that dissolution of carbonates and halite are the other major chemical processes, in addition to cation exchange, that control the groundwater chemistry in the Kisumu aquifer. Hierarchical cluster analysis of the main cations and anions discriminated satisfactorily the various water types in the study area.

The precise mechanisms of SDF-1 (CXCL12) in angiogenesis are not fully elucidated. Recently, we showed that Notch inhibition induces extensive intussusceptive angiogenesis by recruitment of mononuclear cells and it was associated with increased levels of SDF-1 and CXCR4. In the current study, we demonstrated SDF-1 expression in liver sinusoidal vessels of Notch1 knockout mice with regenerative hyperplasia by means of intussusception, but we did not detect any SDF-1 expression in wild-type mice with normal liver vessel structure. In addition, pharmacological inhibition of SDF-1/CXCR4 signalling by AMD3100 perturbs intussusceptive vascular growth and abolishes mononuclear cell recruitment in the chicken area vasculosa. In contrast, treatment with recombinant SDF-1 protein increased microvascular density by 34% through augmentation of pillar number compared to controls. The number of extravasating mononuclear cells was four times higher after SDF-1 application and two times less after blocking this pathway. Bone marrow-derived mononuclear cells (BMDC) were recruited to vessels in response to elevated expression of SDF-1 in endothelial cells. They participated in formation and stabilization of pillars. The current study is the first report to implicate SDF-1/CXCR4 signalling in intussusceptive angiogenesis and further highlights the stabilizing role of BMDC in the formation of pillars during vascular remodelling.

Theileria parva causes East Coast fever (ECF), one of the most economically important tick-borne diseases of cattle in sub-Saharan Africa. A live immunisation approach using the infection and treatment method (ITM) provides a strong long-term strain-restricted immunity. However, it typically induces a tick-transmissible carrier state in cattle and may lead to spread of antigenically distinct parasites. Thus, understanding the genetic composition of T. parva is needed prior to the use of the ITM vaccine in new areas. This study examined the sequence diversity and the evolutionary and biogeographical dynamics of T. parva within the African Great Lakes region to better understand the epidemiology of ECF and to assure vaccine safety. Genetic analyses were performed using sequences of two antigen-coding genes, Tp1 and Tp2, generated among 119 T. parva samples collected from cattle in four agro-ecological zones of DRC and Burundi.

BACKGROUND: To determine the prevalence and causes of blindness and vision impairment (VI) in East Asia in 2015 and to forecast the trend to 2020.
METHODS: Through a systematic literature review and meta-analysis, we estimated prevalence of blindness (presenting visual acuity <3/60 in the better eye), moderate-to-severe vision impairment (MSVI; 3/60≤presenting visual acuity <6/18), mild vision impairment (mild VI: 6/18≤presenting visual acuity <6/12) and uncorrected presbyopia for 1990, 2010, 2015 and 2020. A total of 44 population-based studies were included.
RESULTS: In 2015, age-standardised prevalence of blindness, MSVI, mild VI and uncorrected presbyopia was 0.37% (80% uncertainty interval (UI) 0.12%-0.68%), 3.06% (80% UI 1.35%-5.16%) and 2.65% (80% UI 0.92%-4.91%), 32.91% (80% UI 18.72%-48.47%), respectively, in East Asia. Cataract was the leading cause of blindness (43.6%), followed by uncorrected refractive error (12.9%), glaucoma, age-related macular degeneration, corneal diseases, trachoma and diabetic retinopathy (DR). The leading cause for MSVI was uncorrected refractive error, followed by cataract, age-related macular degeneration, glaucoma, corneal disease, trachoma and DR. The burden of VI due to uncorrected refractive error, cataracts, glaucoma and DR has continued to rise over the decades reported.
CONCLUSIONS:Addressing the public healthcare barriers for cataract and uncorrected refractive error can help eliminate almost 57% of all blindness cases in this region. Therefore, public healthcare efforts should be focused on effective screening and effective patient education, with access to high-quality healthcare.

There are 2.7 ophthalmologists per million population in sub-Saharan Africa, and a need to train more. A link to a web-based questionnaire was sent to all heads of eye departments and training programme directors of ophthalmology training institutions in Eastern, Central and Southern Africa, who forwarded to all their trainees and recent graduates. Main outcome measures were quantitative and qualitative survey responses. Responses were obtained from 124 (52%) trainees in the region. Overall level of satisfaction with ophthalmology training programmes was rated as 'somewhat satisfied' or 'very satisfied' by 72%. Most frequent intended career choice was general ophthalmology, with >75% planning to work in their home country post-graduation. A quarter stated a desire to mainly work in private practice. Only 28% of junior (first and second year) trainees felt surgically confident in manual small incision cataract surgery (SICS); this increased to 84% among senior trainees and recent graduates. The median number of cataract surgeries performed by junior trainees was zero. 57% of senior trainees were confident in performing an anterior vitrectomy. Only 29% of senior trainees and 64% of recent graduates were confident in trabeculectomy. The mean number of cataract procedures performed by senior trainees was 84 SICS (median 58) and 101 phacoemulsification (median 0). Satisfaction with post-graduate ophthalmology training in the region was fair. Most junior trainees experience limited cataract surgical training in the first two years. Focused efforts on certain aspects of surgical education should be made to ensure adequate opportunities are offered earlier on in ophthalmology training.

Here we outline the basis on which a formal proposal should be made for potential inclusion of the Anthropocene in the Geological Time Scale, examining the scale and rate of human change to the Earth System to help recognise the point at which anthropogenic impacts became of sufficient scale to allow discrimination of the Anthropocene as a geological unit. This examination covers such factors as impacts from early hominin species, the first human artefacts, early ecosystem modification through agriculture, deforestation, the domestication of animals, urbanisation, metal mining and smelting and early globalisation. The Industrial Revolution, starting in the UK in the 18th century, and the global Great Acceleration of the mid-20th century, are investigated, as both provide popular narratives that explain the Earth System changes indicative of the Anthropocene, with the latter producing the near-synchronous stratigraphic signals most consistent with an effective geological time boundary. We assess which hierarchical level–age, epoch, period, era or eon–seems most suitable for the Anthropocene, and suggest that epoch (= series) level is conservative and appropriate. The Anthropocene might be defined via a Global Standard Stratigraphic Age or a Global boundary Stratotype Section and Point, with the latter being most appropriate. Finally, we assess the kinds of geological environments, including anoxic marine basins, annually banded coral and bivalve skeletons, estuaries and deltas, lake floors, peat mires, anthropogenic deposits, polar ice, speleothems and tree rings, in which such a physical reference level might be placed.

Data collection in life table experiments is generally time-consuming and costly such that data reconstruction of published information provides an avenue to access the original data for purposes of further investigation. In this paper, we present an algorithm that reconstructs life table raw data using a summary of results from published articles. We present the steps of the development and implementation (in the R computer language) of the algorithm, its scope of application, assumptions, and limitations. Statistical background of the algorithm is also presented. The developed algorithm was then applied to reconstruction of life table data of two insect species, Chilo partellus and Busseola fusca, from published information. Welch’s two-sample t-test was applied to test the difference between the original and reconstructed data of the insect life stages. C. Partellus results were not significantly different, but, for B. fusca, pupa development time, and larva and pupa development rate were significantly different at the 95% confidence level. It is concluded that the algorithm could be used to reconstruct original data sets from cohort life table data sets of insects, given published information and sample sizes.

This is because surface water is both diminishing in quantity due to climate variability and deteriorating in quality due to high levels of anthropogenic contamination. In the quest to attain the Sustainable Development Goal number 6 that aim at ensuring access to safe water by all by 2030, the Government of Kenya is encouraging the development of groundwater resources whose potential is enormous though it has not been quantified. The Inter-governmental Authority on Development (IGAD) supported this research on the shared Mt. Elgon trans-boundary aquifer between Kenya and Uganda aimed at understanding its dynamics. Mt. Elgon is a Tertiary age mountain that straddles the Kenya-Uganda border and has a trans-boundary aquifer. This study investigated the groundwater chemistry and its implication on water management and human health. Physico-chemical parameters of water that included electrical conductivity, pH, and temperature were measured in the field and the major cations and anions were measured at the Central Laboratories of the State Department for Water. Geological mapping and identification of sanitary risks were undertaken during the field work. The study revealed that the concentration of cations and anions in the groundwater varied spatially and temporally. Abundance of these ions were in the order Ca²⁺ > Na⁺ > Mg²⁺ > K⁺ for most samples and HCO₃⁻ > Cl⁻ > SO₄²⁻ >NO₃⁻. Interpretation of hydro-chemical data suggests that calcium carbonate dissolution, halite dissolution, Ca/Na ion exchange and Mg/Na ion exchange are the major processes that control the ground-water chemistry. Chemical results indicate further that the groundwater is suitable for domestic use but is threatened by both anthropogenic and geological factors. Extensive use of fertilizer and the destruction of the catchment area coupled with low permeability and rock-water interactions in the metamorphic rock terrains are the main threats to groundwater quality in the region. A few water points had water with some ionic composition exceeding WHO and the local KEBS maximum limits for drinking water. Such water pose a risk to human health.

The first phytochemical investigation of the flowers of Millettia dura resulted in the isolation of seven isoflavones, a flavonol and a chalcone. Background: Timely identification and management of intrapartum complications could significantly reduce maternal deaths, intrapartum stillbirths, and newborn deaths due to hypoxia. The World Health Organization (WHO) identifies monitoring of labor using the paper partograph as a high-priority intervention for identifying abnormities in labor and fetal well-being. This article describes a mixed-method, quasi-experimental study to assess the effectiveness of an Android tablet-based electronic, labor clinical decision-support application (ePartogram) in limited-resource settings.

Methods: The study, conducted in Kenya from October 2016 to May 2017, allocated 12 hospitals and health centers to an intervention (ePartogram) or comparison (paper partograph) group. Skilled birth attendants (SBAs) in both groups received a 2-day refresher training in labor management and partograph use. The intervention group received an additional 1-day orientation on use and care of the Android-based ePartogram app. All outcomes except one compare post-ePartogram intervention versus paper partograph controls. The exception is outcome of early perinatal mortality pre- and post-ePartogram introduction in intervention sites compared to control sites. We used log binomial regression to analyze the primary outcome of the study, suboptimal fetal outcomes. We also analyzed for secondary outcomes (SBAs performing recommended actions), and conducted in-depth interviews with facility in-charges and SBAs to ascertain acceptability and adoptability of the ePartogram.

Results: We compared data from 842 clients in active labor using ePartograms with data from 1,042 clients monitored using a paper partograph. SBAs using ePartograms were more likely than those using paper partographs to take action to maintain normal labor, such as ambulation, feeding, and fluid intake, and to address abnormal measurements of fetal well-being (14.7% versus 5.3%, adjusted relative risk=4.00, 95% confidence interval [CI]=1.95–8.19). Use of the ePartogram was associated with a 56% (95% CI=27%–73%) lower likelihood of suboptimal fetal outcomes than the paper partograph. Users of the ePartogram were more likely to be compliant with routine labor observations. SBAs stated that the technology was easy to use but raised concerns about its use at high-volume sites. Further research is needed to evaluate costs and benefit and to incorporate recent WHO guidance on labor management.

Conclusion: ePartogram use was associated with improvements in adherence to recommendations for routine labor care and a reduction in adverse fetal outcomes, with providers reporting adoptability without undue effort. Continued development of the ePartogram, including incorporating new clinical rules from the 2018 WHO recommendations on intrapartum care, will improve labor monitoring and quality care at all health system levels.

Maize, wheat, and rice are major cereal crops in the world, which provide more caloric energy than any other type of cereal class. In recent years, the yield of these cereal grains has stagnated due to a myriad of constraints, including infestation by pests and pathogens. Plant pathogens, especially agricultural phytopathogenic fungi, may be resistant to the available chemical fungicides which may have detrimental effects and are expensive. This study investigated whether natural products isolated from Linzia glabra Steetz of family Asteraceae could be used as alternative antifungals for control of cereal diseases caused by agricultural fungi. A single pure fraction was separated and identified from L. glabra methanolic flower crude extract using high performance liquid chromatography and high resolution mass spectrometry methods. The pure fraction isolated was tested for inhibition of spore germination in vitro against Fusarium graminearum, Fusarium oxysporum, Fusarium verticillioides, Zymoseptoria tritici, Ustilago maydis, and Pyricularia oryzae. The fraction was identified as a flavonol, 3-O-Methylquercetin using high resolution mass spectrometry. Among the fungi tested, spores of Z. tritici were the most susceptible to 3-O-Methylquercetin (44% spore inhibition), followed by F. oxysporum (33% inhibition) at 27 μg/mL. Azoxystrobin, a standard commercialized fungicide, gave a complete inhibition of 100% against Z. tritici and 58% inhibition against F. oxysporum at 5 μg/mL. L. glabra contains flavonols with antifungal activity, but the modest and weak antifungal activity of 3-O-Methylquercetin compared to a standard such as azoxystrobin demonstrates the challenge to produce highly potent natural product fungicides. Abstract
This paper presents results from eight field studies in Asia and Africa on the emissions performance of 16 stove/fuel combinations measured during normal cooking events in homes. Characterizing real-world emissions performance is important for understanding the climate and health implications of technologies being promoted as alternatives to displace baseline cooking stoves and fuels. Almost all of the stove interventions were measured to have substantial reductions in PM2.5 and CO emissions compared to their respective baseline technologies (reductions of 24–87% and 25–80%, for PM2.5 and CO emission rates, respectively), though comparison with performance guidance from the World Health Organization (WHO) and the International Organization for Standardization (ISO) suggests that further improvement for biomass stoves would help realize more health benefits. The emissions of LPG stoves were generally below the WHO interim PM2.5 emissions target (1.75 mg/min) though it was not clear how close they were to the most aspirational ISO (0.2 mg/min) or WHO (0.23 mg/min) targets as our limit of detection was 1.1 mg/min. Elemental and organic carbon emission factors and elemental-to-total carbon ratios (medians ranging from 0.11 to 0.42) were in line with previously reported field-based estimates for similar stove/fuel combinations. Two of the better performing forced draft stoves used with pellets—the Oorja (median ET/TC = 0.12) and Eco-Chula (median ET/TC = 0.42)—were at opposite ends of the range, indicating that important differences in combustion conditions can arise even between similar stove/fuel combinations. Field-based tests of stove performance also provide important feedback for laboratory test protocols. Comparison of these results to previously published water boiling test data from the laboratory reinforce the trend that stove performance is generally better during controlled laboratory conditions, with modified combustion efficiency (MCE) being consistently lower in the field for respective stove/fuel categories. New testing approaches, which operate stoves through a broader range of conditions, indicate potential for better MCE agreement than previous versions of water boiling tests. This improved agreement suggests that stove performance estimates from a new ISO laboratory testing protocol, including testing stoves across low, medium, and high firepower, may provide more representative estimates of real-world performance than previously used tests. More representative results from standardized laboratory testing should help push stove designs toward better real-world performance as well as provide a better indication of how the tested technologies will perform for the user. This article has critically analyzed the influence of United Nations Security Council in the management of
international conflict in the 21st Century. Overtime, conflict has become a recurring decimal in human affairs
globally. It was subsequently to assume a wider and more devastating dimension that an urgent need for man to
find a panacea arose, or else it will threaten the corporate existence of mankind. It is in this view that deliberate
efforts were adopted to checkmate the increasing adverse effect of conflict. An aggregation of the efforts put in by
man in this regard is what is known as conflict management. Part of these efforts is the establishment of
international bodies to ensure global peace and security. The first of these bodies is the League of Nations, which
was established after the First World War. Unfortunately, the League of Nations was unable to prevent a second
world war. This led to its disbandment and the subsequent establishment of the United Nations. The United
Nations organ that superintends over international peace and security is the United Nations Security Council. A
cursory look at the activities of the United Nations Security Council in the 21st Century revealed that though the
organ had recorded a couple of failures, the successes recorded outweigh the failures. For instance, the activities
of the Council have ensured that at least the world has been saved from the agony of the scourge of a third world
war. That in itself among so many other things is quite commendable. Even the failures of the United Nations
Security Council often cited are as a result of the organization of the Council, which allows for five members to
be permanent and these permanent members are clothed with veto powers. The article contended that this veto
power conferred on its wielders an overarching influence on the Council, which in the long run constituted an
encumbrance to the effectiveness and efficiency of the Council. A couple of these successes and failures though in
exhaustive, were succinctly brought out in the article. In order to maximize the efficiency and effectiveness of the
Council, the article proffered a way forward. Top among the recommendations on the way forward is the
restructuring of the United Nations Security Council to make it more inclusive and democratic.

Safe water of adequate quantity, and dignified sanitation, is vital for the sustenance of a healthy and productive human population. In the recognition of this, the United Nations formulated the Sustainable Development Goal No. 6 to ensure access to safe water and sanitation by all by 2030. Actualization of this Goal requires information on the existing status of water resources and sanitation levels. Knowledge on contamination of groundwater is essential to prevent risks to human health. The objective of this study was to determine groundwater contamination in Kisumu, Kenya. A total of 275 water samples were collected from 22 sites within the informal settlements between December 2016 and December 2017. The samples were analysed for bacterial contamination and physical chemical quality. Thermal tolerant coliform bacteria enumeration was used as a proxy to bacteria contamination, and the pH, turbidity, dissolved oxygen, conductivity, salinity and temperature were used as physical chemical indicators of contamination. The results indicate that groundwater in Kisumu hosed coliform bacteria and therefore didn’t comply with contamination limits for domestic water proposed by WHO and local KEBS standards. The results further indicated that the levels of bacteriological contamination vary with water type, shallow well having the highest bacterial loads. The study concluded that there were potential risks to human health due to high content of coliform bacteria. The study attributed the contribution to pit latrines that were present in virtually all compounds. The pit latrines are located close to the water points. The study recommended the definition of minimum distance between the pit latrines and shallow wells to minimize contamination. The low income dwellers should be educated on simple ways of treating drinking water contaminated by microbial to minimize enteric infections.
